Dear Joyce and William Hogan:The Division of Drinking Water (the Division) received a request on October 25, 2024, for an Operating Permit for the Silver Avenue water line extension
project. The Division received as-built plans and a PE Certification of Rule Conformance for the project on November 14, 2024. The Division issued Plan Approval for this project on January
1, 2022.
Our understanding of the project is the installation of approximately 860 linear feet of 8-inch diameter PVC (C900) water line with associated valves and appurtenances. The project included
installing two (2) new fire hydrants and adding three (3) new service connections. The new water line was connected to the existing 8-inch diameter line along Silver Avenue
We have received the following information for the Silver Avenue water line extension:
Certification of Rule Conformance with plan approval conditions by a professional engineer.
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Silver Avenue water line extensionis hereby issued as constituted
by this letter. You may now place the Silver Avenue water line extension in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
