Dear John,

The Church Wells 300,000 Gallon Storage Tank and Water Line project was constructed to the
approved plans and specifications and any changes made were done in accordance with Utah
Drinking Water Rules as described in Utah Administrative Code, R309-500 through 550.

Due to funding constraints, the booster pump that was originally permitted was not constructed.
However, the booster pump was designed for lots that are not developed at the current time,
and all existing structures meet peak day and peak instantaneous demands.

Operations and maintenance manuals for valves along with bacteriological test results, contact
information, and record drawings of the project are included. Church Wells has a copy of these
items for their records.

An operating permit for this project is requested.
