ENGINEER’S CERTIFICATION AND DECLARATION

I, Jacob M. Clegg, hereby certify I am a Registered Professional Civil Engineer holding
registration number 9447427-2202 in the state of Utah. I declare that all conditions of plan
approval were accomplished for the South Rim Water Line Extension along Silver Avenue,
South Rim Subdivision, System #23079, File #12833 per the plan approval dated January 11,
2022. As-built “record” drawings submitted to the Division of Drinking Water which Ensign
prepared on September 19th, 2024 and provide to the Owner and submitted to the Division on
October 25th, 2024. I also certify the project is in conformance with the State of Utah Division
of Drinking Water Rules and Regulations and that the water system owner has received a copy of
the as-built “record” drawing.
