JACKS AUTO – RELEASE ID FQU 103 EAST CENTER STREET, MIDVALE UT 1 INTRODUCTION AND BACKGROUND ____________________________________________________________________________ This report presents the results of the groundwater monitoring activities performed at the former Jack’s Southwest Collision (Jack’s Auto) Release Site FQU in Midvale, Utah on behalf of the Utah Department of Environmental Quality (UDEQ or Client). Groundwater samples were collected on April 11-12 and April 27, 2023, from ten (10) of the fifteen (15) groundwater monitoring wells proposed for sampling under Work Assignment FQU-14-KLE and one (1) additional monitoring well, MW-5, approved for sampling under Work Assignment FQU-14-KLE-B. Six (6) of the ten (10) proposed monitoring wells for LNAPL recovery testing were monitored. Groundwater sampling and Product testing were conducted in accordance with the UDEQ Division of Environmental Response and Remediation (DERR) requirements by a Utah Certified Soil and Groundwater Sampler, under the direction of Utah Professional Geologists and a Utah Certified UST Consultant. The groundwater samples were collected from the existing monitoring wells to assess petroleum hydrocarbon concentrations in groundwater at the Site and in the downgradient groundwater flow direction, in accordance with Utah Groundwater Quality Protection Rule R317-6-6.15. The Site location is indicated on Figure 1. Figure 2 depicts the current Site layout and groundwater monitoring well locations with the associated groundwater elevations. 1.1 SITE DESCRIPTION The Site is a former autobody shop which operated four underground storage tanks (USTs). In 1991, the four USTs were removed from the Site. In 1989 and 1990, UST removal subsurface investigation work was conducted at the west adjacent former Handimart property (now known as La Coqueta). Petroleum releases affecting soil and groundwater at the two properties were reported to the UDEQ in 1991 and 1992. Sometime after 1991, a soil vapor extraction (SVE) system was installed at the former Handimart property. 20235345.001A/SLC23R155803 Page 2 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 From 1993 through the present-day, additional groundwater monitoring wells, piezometers, and soil borings were advanced at the Site and nearby properties to assess the plume. In 1993, Light Non-Aqueous Phase Liquid (LNAPL or product) up to 6.6 feet thick was observed in several groundwater monitoring wells and piezometers. In 1995, an LNAPL recovery system was installed at the Site. The LNAPL recovery system was only operated for approximately one month due to limited LNAPL recharge and recovery. Following shutdown of the LNAPL recovery system, bailing was the primary method used for LNAPL recovery. 20235345.001A/SLC23R155803 Page 3 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 2 GROUNDWATER AND PRODUCT MONITORING ____________________________________________________________________________ 2.1 GROUNDWATER GAUGING On April 11 and 27, 2023, Kleinfelder personnel assessed the general condition of and gauged twenty-five (25) groundwater monitoring wells. Four (4) of the proposed fifteen twenty-five (25) groundwater monitoring wells were dry and therefore samples could not be collected. Kleinfelder field personnel used an oil/water interface probe (OIP) to measure depth to product, depth to groundwater, and total depth of each groundwater monitoring well. The OIP was decontaminated using a non-phosphate wash solution and a distilled water rinse prior to use and between each well. 2.2 GROUNDWATER SAMPLING AND LAB ANALYSIS On April 11-12 and 27, 2023, Kleinfelder collected groundwater samples from the eleven (11) monitoring wells not containing product with sufficient water volume for sampling. The off-Site monitoring wells that were sampled were selected to assess groundwater quality down/cross gradient of the historical Jack’s Auto property. Prior to sampling, a minimum of three well casing volumes were purged from each groundwater monitoring well using a new factory-sealed disposable bailer and allowed to recover at least 80 percent of the original static water level. Groundwater samples were collected with the disposable bailer dedicated to each well. The purge water was not observed to have a visible sheen and was poured on an intact asphalt surface and allowed to evaporate following sample collection. The groundwater samples were placed in laboratory-supplied containers, labeled, immediately stored on ice, and transported under chain-of-custody documentation to Pace Analytical National Center for Testing and Innovation (Pace) in Mount Juliet, Tennessee for analysis of methyl tert-butyl ether, benzene, toluene, ethylbenzene, xylenes, and naphthalene (MBTEXN) and total petroleum hydrocarbons (TPH) as gasoline-range organics (TPH-GRO) by Environmental Protection Agency (EPA) Method 8260 and TPH as diesel-range organics (TPH-DRO) by EPA Method 8015. The laboratory groundwater analytical reports and chain-of-custody documentation are included in Appendix A. 20235345.001A/SLC23R155803 Page 4 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 2.3 PRODUCT GAUGING AND RECOVERY The depths to LNAPL (Product) and groundwater were measured on April 26, 2023, in the proposed monitoring wells MW-5, MW-8, MW-12, MW-17, MW-21, MW-23, MW-24, MW-26, and B-4. Once gauging was completed, Kleinfelder used weighted disposable bailers to remove LNAPL from each well. The waste generated from the recovery of LNAPL was placed into a 55-gallon steel drum and picked up for disposal by E.T. Technologies on April 26, 2023. Kleinfelder provided E.T. Technologies with the density and flashpoint results of a waste characterization sample taken from MW-17 on April 12, 2023, for proper disposal. The laboratory report for the waste characterization is included in Appendix A. Kleinfelder returned to the monitoring wells where LNAPL was removed to document the depths to LNAPL and groundwater one, two, four, seven, and eight weeks from the initial recovery in accordance with Work Assignment FQU-14-KLE. 2.4 DEVIATIONS FROM THE APPROVED WORK PLAN On April 26, 2023, Kleinfelder gauged monitoring well MW-5 for LNAPL recovery testing due to previous detection; however, Kleinfelder personnel did not observe measurable LNAPL in the monitoring well and acquired approval to add the well to the groundwater sampling event. On April 26, 2023, Kleinfelder was unable to access monitoring wells MW-21, MW-24, and B-4 due to the presence of abandoned or lived-in vehicles. Kleinfelder notified the Client of the anticipated deviation, and the monitoring wells were approved to be excluded from the LNAPL gauging and recovery task. 20235345.001A/SLC23R155803 Page 5 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 3 INVESTIGATION RESULTS ____________________________________________________________________________ Gauging data from the April 2023 groundwater monitoring event are presented in Table 1. Groundwater analytical results for the April 2023 groundwater sampling event are presented in Table 2 and summarized below. A potentiometric surface map, using data from the April 2023 gauging event, is included as Figure 2. Figures 3 through 5 present iso-concentration maps for TPH-GRO, TPH-DRO, and benzene, respectively. 3.1 GROUNDWATER GAUGING RESULTS Based on the potentiometric surface map provided in Figure 2, the dominant groundwater flow direction trends outward from MW-17 in all directions at a gradient of approximately 0.05 foot/foot using the lowest groundwater elevation of 4314.42 feet above mean sea level (amsl) in MW-22 and highest groundwater elevation of 4324.74 amsl in MW-17. Additional groundwater gradients calculated from MW-17 and wells to the north, east, and south range from 0.04 to 0.08 foot/foot. Varying groundwater gradients between the monitoring events could be influenced by the difference of LNAPL thickness, the hydrocarbon density in monitoring wells, and the number of dry wells that Kleinfelder observed during the current monitoring event. Depths to groundwater ranged from 35.18 feet below ground surface (bgs) in MW-23 to 47.28 feet bgs in MW-1. Groundwater elevations for each of the twenty-five (25) wells monitored during April 2023 are included on Table 1. The groundwater potentiometric surface is presented in Figure 2. Groundwater elevations were corrected for the presence of LNAPL using the formula: PCWL = (TOC – WD) + (WD-PD) * (H Density / W Density) Where PCWL is the product corrected water level, TOC is the top of casing elevation, WD is the depth to water, PD is the depth to product, H Density is the hydrocarbon density of 0.80 grams per milliliter (g/ml), and W Density of the water density of one g/ml. The hydrocarbon density of 0.80 g/ml was selected based on the waste characterization result collected from MW-17 on April 12, 2023, and analyzed by Pace. The laboratory report for the waste characterization is included in Appendix A. 20235345.001A/SLC23R155803 Page 6 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 3.2 GROUNDWATER ANALYTICAL RESULTS The following presents a summary of the groundwater analytical results presented in Table 2 for the monitoring wells sampled in April 2023. Historical analytical results containing the November 2022 analytical results are included in Table 3. Concentrations of TPH-GRO were reported above the UDEQ DERR Initial Screening Level (ISL) of one milligram per liter (mg/L) in the groundwater sample collected from on-Site monitoring wells MW-4 (reported at 8.73 mg/L). In addition to exceeding the ISL, on-Site monitoring wells MW-5 and MW-7 exceeded the UDEQ DERR Tier 1 Screening Level (SL) of 10 mg/L (reported at 23.1 and 11.9 mg/L, respectively). Concentrations of TPH-DRO were reported above the UDEQ DERR ISL of one mg/L in the groundwater sample collected from on-Site groundwater monitoring wells MW-4 (reported at 3.17mg/L), MW-5 (reported at 7.09 mg/L), MW-6 (reported at 1.95 mg/L), and MW-7 (reported at 6.11 mg/L). In addition to exceeding the ISL, the duplicate sample MW-35, taken from monitoring well MW-5, exceeded the UDEQ DERR Tier 1 SL of 10 mg/L (reported at 11.1 mg/L). Benzene was reported at concentrations exceeding the UDEQ DERR ISL of 0.005 mg/L in the groundwater samples collected from on-site monitoring well MW-4 (reported at 0.0288mg/L), MW-5 (reported at 0.162 mg/L), and MW-7 (reported at 0.0930 mg/L). No concentrations of benzene were reported above the UDEQ DERR Tier 1 SL of 0.3 mg/L. Toluene, ethylbenzene, total xylenes, methyl tert-butyl ether, and naphthalene were not reported in groundwater samples collected from the on or off-Site monitoring wells at concentrations above the UDEQ DERR SLs. Isoconcentration maps of TPH-GRO, TPH-DRO, and benzene were drawn using the Surfer® groundwater modelling application, and are included as Figures 3 through 5, respectively. 3.3 QUALITY ASSURANCE AND QUALITY CONTROL Pace reported the qualifier “B” for TPH-DRO in monitoring wells MW-2, MW-3, and MW-13, indicating TPH-DRO was detected in the associated laboratory method blank. The qualifier “B” indicates that the results for TPH-DRO in these monitoring wells could be biased high. The reported concentrations have been compared to the results from the November 2022 monitoring event and are generally similar. No additional qualifiers were reported by Pace; therefore, the data is considered accurate. 20235345.001A/SLC23R155803 Page 7 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 Two (2) duplicate samples were collected for quality assurance (QA) purposes, DUP-1 from MW-6 and MW-35 from MW-5. The Environmental Protection Agency (EPA) QA guidelines establish that the range of quality in precision (the relative percent difference or RPD) between two measurements should not exceed 20 percent for aqueous samples. TPH-DRO in MW-35 and MW-5 exceeded the EPA guideline for precision with an RPD of 44.1. Benzene in DUP-1 and MW-6 exceeded the EPA guideline for precision with an RPD of 28.9. The majority of RPDs for the parent and duplicate samples were within the EPA guideline for precision in the majority of the analyses; therefore, the results are considered sufficiently precise. The laboratory report from Pace is included in Appendix A, and the RPDs are presented in Table 4. 3.4 LNAPL RECOVERY RESULTS Measurable LNAPL thickness was present in six (6) monitoring wells MW-8, MW-12, MW-17, MW-20, MW-23, and MW-26. As presented in Table 5, the LNAPL thickness prior to the recovery ranged from 0.13 feet in MW-23 to 1.66 feet in MW-17. The depth to groundwater decreased over the eight (8) weeks by 0.21 to 2.22 feet. The results from the product recovery testing from the six (6) monitoring wells with measurable LNAPL in April 2023 are presented in Table 5 and summarized below: • LNAPL thickness in monitoring well MW-8 was observed to be 0.55 feet in the initial gauging event prior to recovery (Week 0) and gradually recovered to 0.47 feet in Week 8, approximately 85% of the original thickness. • LNAPL thickness in monitoring well MW-12 was observed to be 0.57 feet in Week 0 and gradually recovered to 0.38 feet in Week 8, approximately 67% of the original thickness. • LNAPL thickness in monitoring well MW-17 was observed to be 1.66 feet in Week 0 and gradually recovered to 1.06 feet, approximately 64% of the original thickness. • LNAPL thickness in monitoring well MW-20 was observed to be 1.58 feet in Week 0. LNAPL thickness fluctuated from 28% of the original thickness in Week 1, decreased to 8.9% of the original thickness in Week 2, then increased to 17% and then 32% in Weeks 4 and 7, respectively, and then decreased to 25% of the original thickness (0.39 feet) in Week 8. 20235345.001A/SLC23R155803 Page 8 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 • LNAPL thickness in monitoring well MW-23 was observed to be 0.13 feet in Week 0. LNAPL thickness fluctuated from 23% of the original thickness in Week 1, then decreased to a non-measurable thickness of LNAPL in Week 2, then increased to 31% and then 62% in Weeks 4 and 7, respectively, and then decreased to 38% of the original thickness (0.05 feet) in Week 8. • LNAPL thickness in monitoring well MW-26 was observed to be 0.50 feet in Week 0. LNAPL thickness fluctuated from 38% of the original thickness in Week 1, then increased to 40% in Weeks 2 and 4, then decreased to 28% in Week 7, and increased to 30% of the original thickness (0.15 feet) in Week 8. The LNAPL gauging and recovery results are included in Table 5. Bar charts depicting the LNAPL recovery over time are included in Figures 6 through 8. 20235345.001A/SLC23R155803 Page 9 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 4 CONCLUSIONS AND RECOMMENDATIONS ____________________________________________________________________________ 4.1 CONCLUSIONS Petroleum hydrocarbon impacts above the UDEQ DERR ISLs and Tier 1 SLs have been reported in groundwater samples collected from the Jack’s Auto Site and in the hydraulically downgradient, off-Site wells. The dominant groundwater flow direction trends radially outward from MW-17 at a gradient of approximately 0.05 ft/ft using the lowest groundwater elevation of 4314.42 feet amsl in MW-22 and the highest groundwater elevation of 4324.74 feet amsl in MW-17. Based on a comparison of the November 2022 monitoring event, the wells exceeding the UDEQ DERR Tier 1 SLs is consistent. Based on a comparison of wells exceeding the UDEQ DERR ISLs from the November 2022 monitoring event, the groundwater contaminant plume has generally remained stable with the exception of TPH-DRO migrating to the east (Kleinfelder, 2022). Monitoring wells MW-8, MW-12, and MW-17 recovered 64-84% of their original LNAPL thickness within the eight-week recovery test period, indicating that removal of LNAPL in these wells may provide meaningful reduction in LNAPL mass at the Site. Monitoring wells MW-20, MW-23, and MW-26 recovered 25-38% of their original LNAPL thickness and fluctuated throughout the eight-week recovery test period, indicating that LNAPL removal from these wells may not provide an efficient reduction in LNAPL mass at the Site. Groundwater fluctuations throughout the eight-week LNAPL recovery test period may also have influenced the LNAPL mobility and recovery in wells such as MW-20 and MW-23, which recorded an increase in groundwater elevations by approximately two feet. 4.2 RECOMMENDATIONS Kleinfelder recommends that the quarterly gauging and sampling of all wells be continued to further evaluate seasonal fluctuations and exceedances of petroleum hydrocarbon screening levels and monitor the perimeter of the free product plume. Kleinfelder also recommends installing additional groundwater monitoring wells to the west and north of the plume to acquire screening data on geologic factors which influence the recovery and mobilization of the groundwater contaminant plume. 20235345.001A/SLC23R155803 Page 10 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 In addition, Kleinfelder recommends using bailers for the regular manual extraction of LNAPL in monitoring wells MW-8, MW-17, and MW-12 due to the relatively rapid product recovery rate out of the six (6) monitoring wells tested. Extraction using passive skimmers or pneumatic sippers was considered for removal; however, manual LNAPL removal is projected to cost less due to the materials costs of passive recovery systems. Following DEQ’s review of this report, Kleinfelder recommends that a meeting be held to discuss the findings and recommendations to finalize the path forward for this Site. 20235345.001A/SLC23R155803 Page 11 of 12 July 11, 2023 © 2023 Kleinfelder www.kleinfelder.com 849 West Levoy Drive, Suite 200, Taylorsville, UT 84123-2544 p | 801.261.3336 5 LIMITATIONS ____________________________________________________________________________ This work was performed in a manner consistent with that level of care and skill ordinarily exercised by other members of Kleinfelder’s profession practicing in the same locality, under similar conditions and at the date the services are provided. Failure to do so will vitiate Kleinfelder’s recommendations.” Kleinfelder offers various levels of investigative and engineering services to suit the varying needs of different clients. It should be recognized that definition and evaluation of geologic and environmental conditions are a difficult and inexact science. Judgments leading to conclusions and recommendations are generally made with incomplete knowledge of the subsurface conditions present due to the limitations of data from field studies. Although risk can never be eliminated, more detailed and extensive studies yield more information, which may help understand and manage the level of risk. Since detailed study and analysis involves greater expense, our clients participate in determining levels of service that provide adequate information for their purposes at acceptable levels of risk. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description B The same analyte is found in the associated blank. J The identification of the analyte is acceptable; the reported value is an estimate. J2 Surrogate recovery limits have been exceeded; values are outside lower control limits. J3 The associated batch QC was outside the established quality control range for precision. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions MDL Method Detection Limit. ND Not detected at the Reporting Limit (or MDL where applicable). RDL Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis.
