left onto 3400 West as the road bends to the northeast. The pit
entrance is on the left (North).

OPERATING STATUS: The asphalt and concrete plants had shut down for the day and the
crushing circuit was down for repair.
PROCESS DESCRIPTION: Aggregate plant - Raw material is moved into the jaw crusher and
material is conveyed to a drop pile off the hillside. Other crushing
circuits screen and crush rock material to desired size. The material is
stacked according to size, and loaded to haul trucks as needed.
Asphalt plant - Course sand and aggregate materials are dumped into
the hoppers. The materials are then fed onto a conveyor belt and
enter an oil-fired drum dryer. Lime may also be added if required by
job specifications. Emissions from the dryer are vented through a
horizontal cyclone collector and then pass through a baghouse. The
liquid asphalt cement (AC-10) is added in the post drum mixer (with
RAP if used). The material is then transported up a short conveyor to
a storage silo. Trucks are loaded from the silo.
Batch plant - Course sand and aggregate materials are dumped into
bins. The materials are conveyed to the batch plant building, where
they are weighed and mixed with cement powder and fly ash. The
material drops into haul trucks below and mixes enroute to the job
site. Emissions from the cement and fly ash silos are vented through
baghouses.

Section I: GENERAL PROVISIONS
I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the
UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to
those rules. [R307-101]
I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401]
I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions
covered by this AO must be reviewed and approved. [R307-401-1]
I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by
the owner/operator, shall be made available to the Director or Director's representative upon
request, and the records shall include the two-year period prior to the date of the request. Unless
otherwise specified in this AO or in other applicable state and federal rules, records shall be kept
for a minimum of two (2) years. [R307-401-8]
I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall,
to the extent practicable, maintain and operate any equipment approved under this AO, including
associated air pollution control equipment, in a manner consistent with good air pollution control
practice for minimizing emissions. Determination of whether acceptable operating and
maintenance procedures are being used will be based on information available to the Director
which may include, but is not limited to, monitoring results, opacity observations, review of
operating and maintenance procedures, and inspection of the source. All maintenance performed
on equipment authorized by this AO shall be recorded. [R307-401-4]
I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns.

I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150]

I.8 The owner/operator shall submit documentation of the status of construction or modification to
the Director within 18 months from the date of this AO. This AO may become invalid if
construction is not commenced within 18 months from the date of this AO or if construction is
discontinued for 18 months or more. To ensure proper credit when notifying the Director, send
the documentation to the Director, attn.: NSR Section. [R307-401-18]
Status: Out of Compliance. No notification was made concerning equipment installation.
No further action is recommended. In Compliance with the remainder of conditions in this
section. No limits appear to have been exceeded. No modifications have been made to
equipment or process. Records are maintained. Equipment appears to be maintained and
operated properly. No reportable breakdowns have occurred since the previous inspection.
A 2023 emission inventory was submitted to DAQ but is still being processed.

II.A.11 Associated Equipment

Status: In Compliance. No unapproved equipment was observed. The equipment that is
listed in items II.A.2, II.A.3, and II.A.11 are listed on this AO and the La Verkin AO
so that they can move them back and forth as needed. At this site, there were two
cone crushers (1 new), two jaw crushers, and four screens (1 new 5’ x 16’). None of
the emergency generators have operated in the last 12 months.
II.B Requirements and Limitations
II.B.1 Site Wide Requirements
II.B.1.a The owner/operator shall not produce more than the following: A. 600,000 tons of aggregate material per rolling 12-month period, of which no more than 540,000 tons of aggregate material is processed in the onsite equipment per rolling 12-month period. B. 150,000 tons of hot mix asphalt per rolling 12-month period of which, no more than 25% (37,500 tons per rolling 12-month period) can be reclaimed asphalt pavement (RAP). C. 75,000 cubic yards of concrete per rolling 12-month period. [R307-401-8]
II.B.1.a.1 The owner/operator shall:
A. Determine production by scale house records or vendor receipts.
B. Record production on a daily basis
C. Use the production data to calculate a new rolling 12-month total by the 20th day
of each month using data from the previous 12 months
D. Keep the production records for all periods the plant is in operation. [R307-401-8]
Status: In Compliance. The 12-month production totals are as follows:

II.B.1.b Unless otherwise specified in this AO, the owner/operator shall not allow visible emissions to
exceed the following limits:
A. Crushers - 12% opacity
B. Screens - 7% opacity
C. All Conveyor Transfer Points - 7% opacity
D. Concrete Batch Plants - 7% opacity
E. All Diesel Engines - 20% opacity
F. All Conveyor Drop Points - 20% opacity
G. All Other Points - 20% opacity
H. All Baghouses/Bin vents - 10% opacity

II.B.1.b.1 Opacity observations of visible emissions from stationary sources shall be conducted according
to 40 CFR 60, Appendix A, Method 9. [40 CFR 60 Subpart OOO, R307-401-8]
Status: Not Observed. Asphalt and Concrete production had finished for the day and the
crushing circuit was down for repair, so opacity observations were not conducted on these
points.
II.B.1.c The owner/operator shall not conduct aggregate processing, offsite hauling, drilling, bulldozing,
or loader operations for more than 3,910 hours each per rolling 12-month period. [R307-401-8]
II.B.1.c.1 The owner/operator shall:
A. Determine hours of operation by monitoring and maintaining an operations log.
The hours of operation of each bulldozer shall be added together to determine the
total hours of bulldozing. The hours of operation of each loader shall be added
together to determine the total hours of loader operations.
B. Record hours of operation each day
C. Use the hours of operation to calculate a new rolling 12-month total by the 20th day
of each month using data from the previous 12 months
D. Keep hours of operation records for all periods the plant is in operation. [R307-401-8]
Status: In Compliance. For the 12-month period, the total plant operation hours was
2,409.5, combined loader operation hours was 3,522, and bulldozing hours was zero. See
attached records.
II.B.1.d From June 1st thru August 31st, the owner/operator shall not conduct aggregate processing,
offsite hauling, drilling, bulldozing, or loader operations before 5:00 am or after 7:00 pm each
operating day. The owner/operator shall not conduct aggregate processing, offsite hauling,
drilling, bulldozing, or loader operations before 7:00 am or after 7:00 pm each operating day

during the rest of the year. The owner/operator shall not operate the Hurricane Pit on Sundays.

II.B.1.d.1 The owner/operator shall keep and maintain the following records of operation for all periods of
operations:
A. Time aggregate processing, offsite hauling, drilling, bulldozing, and loader operations
begin each operational day
B. Time aggregate processing, offsite hauling, drilling, bulldozing, and loader operations
end each operational day
C. The total hours of operation each day of aggregate processing, offsite hauling,
drilling, bulldozing, and loader operations. [R307-401-8]
Status: In Compliance. Plant and equipment operation are within the set parameters. See
attached operation hours log.
II.B.2 Haul Roads and Fugitive Dust Requirements
II.B.2.a The owner/operator shall not allow visible emissions from any fugitive dust source to exceed
20%. [R307-401-8]
II.B.2.a.1 Opacity observations of fugitive dust from intermittent sources shall be conducted according to
40 CFR 60, Appendix A, Method 9; however, the requirement for observations to be made at 15-
second intervals over a six-minute period shall not apply. The number of observations and the
time period shall be determined by the length of the intermittent source. For fugitive dust
generated by mobile sources, visible emissions shall be measured at the densest point of the
plume but at a point not less than one-half vehicle length behind the vehicle and not less than
one-half the height of the vehicle. [R307-401-8]
Status: In Compliance. Only minimal dust was observed from haul traffic. The main haul
road is paved and operation areas are covered with road base material. The water truck
was in operation during the inspection.
II.B.2.b The combined area occupied by all storage piles on site shall not exceed 35.0 acres.
[R307-401-8]
II.B.2.b.1 To determine compliance with the total acres of the storage piles, the owner/operator shall measure the total area of the storage piles at least once every six months and shall maintain a record of the total acres of the storage piles. Compliance shall be through Global Positioning System (GPS) measurements or aerial photographs. The area of each storage pile shall be added together to obtain the total area of all the storage piles on site. Records of the total acres of the storage piles shall contain the following: A. Date of measurements B. Size of each storage pile on site C. Total acres of all storage piles combined. [R307-401-8] Status: In Compliance. According to measurements taken May 1, 2024, total storage pile area was 4.5 acres. Measurements are scheduled to be taken again November 1, 2024. See attached records.

II.B.2.c The owner/operator shall not exceed 35.0 acres of active disturbed area. [R307-401-8]
II.B.2.c.1 Compliance shall be determined through Global Positioning System (GPS) measurements or
aerial photographs. Reclaimed areas such as those with emerged vegetation or hydro-seeded do
not contribute to the disturbed area totals. [R307-401-8]
Status: In Compliance. According to measurements taken May 1, 2024, the total disturbed
area was 32.1 acres. Measurements are scheduled to be taken again November 1, 2024. See
attached records.
II.B.2.d The owner/operator shall not exceed the following limits:
A. 20 blasting episodes per rolling 12-month period
B. 652 tons of ANFO per rolling 12-month period.
C. An area of 10,000 square feet per blast. [R307-401-8]
II.B.2.d.1 Records of blasts shall be kept for all periods when the plant is in operation. The records shall
include the following:
A. Date of each blast
B. Area of each blast
C. Tons of ANFO used for each blast.
The owner/operators shall use the records to calculate a new rolling 12-month total by the 20th
day of each month using data from the previous 12 months. [R307-401-8]
Status: In Compliance. The source conducted six blasts during the 12-month period, using
a total of 142 tons of ANFO. Blast area ranged from 2,544 – 9,011 square feet. See attached
records.
II.B.2.e The owner/operator shall not conduct blasting operations before 12:00 pm or after 4:00 pm each
day. [R307-401-8]
II.B.2.e.1 The owner/operator shall keep and maintain the following records of operation for all periods of
drilling and blasting:
A. Time blasting operations begin each day
B. Time blasting operations end each day. [R307-401-8]
Status: In Compliance. All blasts were conducted at 12:30 pm or 1:00 pm during the
12-month period. See attached records.
II.B.2.f The owner/operator shall cover all unpaved haul roads and wheeled-vehicle operational areas
with road base material. [R307-401-8]
Status: In Compliance. Unpaved haul roads and operational areas are covered in road
base material.

II.B.2.g The owner/operator shall pave the primary aggregate and asphalt truck haul roads on site. The
total length of all paved haul roads on site shall not be less than 4,406 feet combined.
[R307-401-8]
II.B.2.g.1 The paved road length shall be determined through source records or GPS measurements.
[R307-401-8]
Status: In Compliance. The primary aggregate and asphalt haul roads are paved at a total
length of 5,615 feet. See attached records.
II.B.2.h An operational sweeper and water truck shall be made available during each operating day. The
owner/operator shall sweep and flush with water all the paved haul roads on site to maintain
opacity limits listed in this AO. The owner/operator shall use water application to minimize
emissions from fugitive dust and fugitive emissions sources, including storage piles, unpaved
haul roads, and wheeled-vehicle operational areas. If the temperature is below freezing, the
owner/operator shall continue to sweep the road but may stop flushing the paved haul roads with
water and applying water to fugitive dust and fugitive emissions sources. Flushing the paved haul
road with water and applying water to fugitive dust and fugitive emissions sources shall resume
when the temperature is above freezing. If the haul roads are covered with snow or ice, the
owner/operator may stop sweeping the paved haul roads. Sweeping the paved haul roads shall
resume when the haul roads are cleared from snow and ice. [R307-401-8]
II.B.2.h.1 Records of vacuum sweeping and water application shall be kept for all periods when the plant is
in operation. The records shall include the following items:
A. Date and time treatments were made
B. Number of treatments made and quantity of water applied
C. Rainfall amount received, if any
D. Records of temperature, if the temperature is below freezing
E. Records shall note if the paved haul roads are covered with snow or ice. [R307-401-8]
Status: In Compliance. Water treatments are conducted regularly with the 4,000 gallon
water truck. Records of water treatments are kept by the truck driver and compiled in a
spreadsheet. See attached.
II.B.3 Crushers, Screens, and Conveyors Requirements
II.B.3.a The owner/operator shall install water sprays on each crusher, screen, conveyor transfer point,
and conveyor drop point on site to control emissions. Water sprays shall operate as necessary to
prevent visible emissions from exceeding the opacity limits listed in this AO. [R307-401-8]
Status: In Compliance. Water sprays are installed as directed. Water sprays were not
observed operating due to the crushing circuit being down for repair.
II.B.3.b The owner/operator shall perform monthly periodic inspections to check that water is flowing to
water sprays associated with each crusher, screen, and conveyor. If the owner/operator finds that
water is not flowing properly during an inspection of the water sprays, the owner/operator shall
initiate corrective action within 24 hours and complete corrective action as expediently as

II.B.3.b.1 Records of the water spray inspections shall be maintained in a logbook for all periods when the plant is in operation. The records shall include the following items: A. Date the inspections were made B. Any corrective actions taken C. Control mechanism used if sprays are not operating. [40 CFR 60 Subpart OOO, R307-401-8] Status: In Compliance. The source conducts monthly water spray inspections. See attached record.
II.B.3.c The owner/operator shall conduct an initial performance test for all crushers, screens, and
conveyor transfer points on site within 60 days after achieving maximum production rate but not
later than 180 days after initial startup. Performance tests shall meet the limitations specified in
Table 3 of Subpart OOO. Records of initial performance tests shall be kept and maintained on
site for the lifetime of the equipment. [40 CFR 60 Subpart OOO, R307-401-8]
II.B.3.c.1 Initial performance tests for fugitive emission limits shall be conducted according to 40 CFR
60.675(c). The owner/operator may use methods and procedures specified in 40 CFR 60.675(e)
an alternative. [40 CFR 60 Subpart OOO, R307-401-8]
II.B.3.c.2 The owner/operator shall submit written reports of the results of all performance tests conducted
to demonstrate compliance with 40 CFR 60.672 to the Director, attn.: Compliance Section. The
submission shall be postmarked no later than 180 days from the date of this AO or no later than
180 days from equipment start-up, whichever is later. [40 CFR 60 Subpart OOO, R307-401-8]
Status: In Compliance. Initial OOO observations have been conducted. Included in the
attachments are the initial observations for the new cone crusher and screen that are on
site.
II.B.4 Asphalt Plant Requirements
II.B.4.a The owner/operator shall use only natural gas in the drum mix asphalt plant. [R307-401-8]
Status: In Compliance. Only natural gas is used as fuel for the asphalt plant.
II.B.4.b The owner/operator shall use a baghouse to control particulate emissions from the hot mix
asphalt plant on site. This baghouse shall be sized to handle at least 26,000 ACFM for the
existing conditions. All exhaust air from the drum mixer shall be routed through the baghouse
before being vented to the atmosphere. [R307-401-8]
Status: In Compliance. The hot mix asphalt plant emissions are controlled by a baghouse.
II.B.4.c The owner/operator shall install a manometer or magnehelic pressure gauge to measure the static
pressure drop across the hot mix asphalt plant baghouse. [R307-401-8]
II.B.4.c.1 The pressure gauge shall measure the pressure drop in 1-inch water column increments or less.

II.B.4.c.2 The pressure gauge shall be located such that an inspector/operator can safely read the indicator
at any time. [R307-401-8]
Status: In Compliance. A magnehelic is installed as directed.

II.B.4.d At least once every 12 months, the owner/operator shall calibrate the pressure gauges in
accordance with the manufacturer's instructions or replace the gauges. [R307-401-8]
II.B.4.d.1 The owner/operator shall maintain the static pressure drop across the hot mix asphalt plant
baghouse during operation between 2 and 6 inches of water column. [R307-401-8]
Status: In Compliance. The source rotates two magnehelics. One is sent to Asphalt Drum
Rollers in Indiana for calibration while the other is in operation. The last change of
magnehelics took place on January 11, 2024. The asphalt plant was not operating at the
time of inspection so pressure differential was not viewed.
II.B.4.e The owner/operator shall maintain records of the pressure gauge calibrations and replacements.

II.B.4.e.1 The owner/operator shall record the pressure drop at least once per operating day. [R307-401-8]
II.B.4.e.2 The owner/operator shall maintain records of the pressure drop readings. [R307-401-8]
Status: In Compliance. Pressure differential daily readings were provided during the
inspection. All observed readings were within the designated range.
II.B.4.f The owner/operator shall not emit more than the following rates and concentrations from the hot
mix asphalt plant baghouse exhaust stack:

II.B.4.f.1 Compliance Demonstration
To demonstrate compliance with the emission limitations above, the owner/operator shall
perform stack testing on the emissions unit according to the stack testing conditions contained in
this AO. [R307-165-2, R307-401-8]
II.B.4.f.2 Initial Test
The owner/operator shall conduct an initial stack test on the emission unit within 180 days after
startup of the emission unit. [R307-165-2]
II.B.4.f.3 Test Frequency
The owner/operator shall conduct a stack test on the emission unit within three (3) years after the
date of the most recent stack test of the emission unit. The Director may require the
owner/operator to perform a stack test at any time. [R307-165-2, R307-401-8]
II.B.4.g The owner/operator shall conduct any stack testing required by this AO according to the
following conditions. [R307-401-8]
II.B.4.g.1 Notification
At least 30 days prior to conducting a stack test, the owner/operator shall submit a source test

protocol to the Director. The source test protocol shall include the items contained in
R307-165-3. If directed by the Director, the owner/operator shall attend a pretest conference.

II.B.4.g.2 Testing & Test Conditions
The owner/operator shall conduct testing according to the approved source test protocol and
according to the test conditions contained in R307-165-4. [R307-165-4, R307-401-8]
II.B.4.g.3 Access
The owner/operator shall provide Occupational Safety and Health Administration (OSHA)- or
Mine Safety and Health Administration (MSHA)-approved access to the test location.

II.B.4.g.4 Reporting
No later than 60 days after completing a stack test, the owner/operator shall submit a written
report of the results from the stack testing to the Director. The report shall include validated
results and supporting information. [R307-165-5, R307-401-8]
II.B.4.g.5 Possible Rejection of Test Results
The Director may reject stack testing results if the test did not follow the approved source test
protocol or for a reason specified in R307-165-6. [R307-165-6, R307-401-8]
II.B.4.g.6 Test Methods
When performing stack testing, the owner/operator shall use the appropriate EPA-approved test
methods as acceptable to the Director. Acceptable test methods for pollutants are listed below.

A. Temperature - 68 degrees Fahrenheit (293 K)
B. Pressure - 29.92 in Hg (101.3 kPa)

II.B.4.g.10 PM2.5
Filterable PM2.5

The filterable particulate emissions shall be used for compliance demonstration. The condensable
particulate emissions shall not be used for compliance demonstration, but shall be used for
inventory purposes. [R307-401-8]
Status: In Compliance. The August 14, 2024, stack test protocol was reviewed and
accepted by DAQ (DAQC-639-24, dated June 27, 2024). The latest stack test was conducted
August 14, 2024, but results were not yet available. The source conducted stack testing on
August 3, 2021, which failed emission standards. Retesting was conducted after repairs on
September 22, 2021, which resulted in emission rates of 2.4153 lb/hr and 0.0206 gr/dscf.
These results were accepted by DAQ (DAQC-180-22, dated February 10, 2022).
II.B.5 Engine Requirements
II.B.5.a The owner/operator shall not operate:
A. Each emergency engine on site for more than 100 hours per rolling
12-month period during non-emergency situations. There is no time limit on the use
of the engines during emergencies.
B. The 455 kW non-emergency engine on site for more than 3,910 hours per
rolling 12-month period. [R307-401-8]
II.B.5.a.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a new
12-month total by the 20th day of each month using data from the previous 12 months. Records
documenting the operation of the 455 kW generator and each emergency engine shall be kept in
a log and shall include the following:
A. The date each engine was used
B. The duration of operation in hours each engine was used
C. The reason for emergency engine usage for each emergency engine. [40 CFR 63 Subpart
ZZZZ, R307-401-8]
II.B.5.a.2 All engines shall each be equipped with a non-resettable hour meter. The meter shall be installed
such that an inspector/operator can safely read the output at any time. [R307-401-8]
Status: In Compliance. None of the emergency generators have operated in the last 12
months. They are onsite but not connected. The 455 kW non-emergency engine operated
464 hours during the 12-month period. A log of operation hours is kept. The engines are
equipped with non-resettable hour meters.

II.B.5.b The owner/operator shall keep a maintenance plan and records of conducted maintenance and
must, to the extent practicable, maintain and operate the 455 kW (610 hp) engine in a manner
consistent with good air pollution control practice for minimizing emissions. [R307-401-8]
Status: In Compliance. A PM schedule is followed according to the manufacturer's
recommendations for the 455 kW engine and records are kept and were reviewed at the
time of inspection. Annual maintenance and major repairs are conducted by Wheeler.
II.B.5.c From June 1st thru August 31st, the owner/operator shall not operate the 455 kW generator
before 5:00 am or after 7:00 pm each operating day. The owner/operator shall not operate the
455 kW generator before 7:00 am or after 7:00 pm each operating day during the rest of the year.

II.B.5.c.1 The owner/operator shall keep and maintain the following records for all periods of operations:
A. Time operation of the 455 kW generator begins each day
B. Time operation of the 455 kW generator ends each day
C. The total hours of operation each day of the 455 kW generator. [R307-401-8]
Status: In Compliance. The 455 kW engine operates within the set parameters. Hours are
recorded on the crushing circuit controller. Records were provided for review at the time
of inspection.
II.B.5.d The owner/operator shall only use diesel fuel (e.g. fuel oil #1, #2, or diesel fuel oil additives) as
fuel in the 455-kW engine and each emergency generator engine. [R307-401-8]
II.B.5.d.1 The owner/operator shall only combust diesel fuel that meets the definition of ultra-low sulfur
diesel (ULSD), which has a sulfur content of 15 ppm or less. [R307-401-8]
II.B.5.d.2 To demonstrate compliance with the ULSD fuel requirement, the owner/operator shall maintain
records of diesel fuel purchase invoices or obtain certification of sulfur content from the diesel
fuel supplier. The diesel fuel purchase invoices shall indicate that the diesel fuel meets the
ULSD requirements. [R307-401-8]
Status: In Compliance. Diesel fuel is purchased through Rhinehart Oil and is ultra-low
sulfur.
II.B.6 Concrete Batch Plant Requirements
II.B.6.a The owner/operator shall control the PM10 emissions from the concrete batch plant with a
baghouse. [R307-401-8]
Status: In Compliance. No emissions were observed from the concrete batch plant.
Baghouses are installed to control concrete emissions.
II.B.6.b The X-3 Baghouse shall vent no less than 63 feet above ground level. [R307-401-8]
Status: In Compliance. The baghouse stack appeared to be the appropriate height.

II.B.6.c The X-4 baghouse shall vent no less than 41 feet above ground level. [R307-401-8]

According to my records, the Magnehlic was swapped out on January 11, 2024. and January 16, 2023, the year prior. As I explained during
our visit we have two of these and ship one off early January every year and get it calibrated before the swap. Item is sent to Asphalt Drum
Rollers in Indiana for calibration.

Hi Beau,
Just something to note for the future if you add equipment to an AO. Condition I.8 of your AOs require you to notify DAQ
when equipment has been installed. Just an email would satisfy this condition and make sure you don't get penalized.
