Southwest District Engineer UDEQ 620 South 400 East #400 Saint George, UT 84770 Subject:Long Valley 2.5 MG Tank (ST009)and Pump Station (PF0006), Transmission Line; Washington City,System #27021, File #13590;Operating Permit Request Dear Mr. Wright, This letter is being submitted to officially request an operating permit for the Long Valley 2.5 MG Tank (identified as ST009 in the Division’s database)and Booster Pump station (identified as PF0006)project (review file #13590). The Contract Record Drawings (CRDs) are attached. The CRDs have been delivered to Washington City. Washington City has GPS points so that their GIS can be updated in agreement with the CRDs. The air vent at the center of the Long Valley 2.5 MG Tank (ST009)has a latitude and longitude of N 37°06’28.68’’ and W 113°28’05.39’’, respectively. There were minor changes affecting the culinary water system that were made during construction. These changes generally consisted of minor horizontal or vertical adjustments, and in one case, an area of special construction to encase the transmission line where it crossed under an existing sewer line.These changes are shown in the included project Contract Record Drawings. Flushing and disinfection of the tank and all transmission lines have been completed. The disinfection report is attached to this letter. Washington City has received the O&M information for the pump system within the Long Valley Pump Station (PF0006). All changes on the project are in conformance with applicable sections of the Utah Administrative Code (UAC), namely R309-500 through R309-550. In accordance with UAC R309-500-9, I certify that all conditions of plan approval (conditions letter dated February 5, 2024) applicable to the completed improvements for the Long Valley 2.5 MG Tank and Booster Pump Station project, have been accomplished.
