Dear Mr. Smoot, Subject: RTCR Level 2 Assessment Beaver Creek Lodge UTAH03112
I would like to thank you for taking the time to meet with Jake Wood (RuralWater) and myself on
September 19th, 2024, for a Level 2 Assessment (L2) of your water system. Your willingness to accommodate this request is greatly appreciated. Observations:
This site visit was triggered by 2 Level 1 assessments in a 12-month period, Routine and Repeat
TC+ EC- samples October 2023 and September of 2024. LHD collects Routine sample and system collected repeats and source sample. System consists of Well which pumps to Connected Tanks and then gravity feeds to lodge where water is chlorinated as it enters the premise plumbing.
Tanks upgradient from lodge near highway at the time of the assessment had a compromised
vent, 2” PVC was broken, and downturned screened opening was laying on the ground, overflows were missing screens and/or were broken. System Indicated during the October 2023 Incident, their chlorinator had malfunctioned as was not working properly.
Conclusions: 1. TC + samples were likely caused by contamination entering the Storage tanks through the unprotected vent and overflows and the chlorinator malfunctioning at the same time. 2. Storage Tanks are likely dirty and possibly contain foreign matter from the unprotected
openings.
Deficiencies: See Deficiency Report, Significant Deficiencies must be corrected within 30 days.

Recommendations:
1. Follow RTCR and Ground Water Rule with regards to Repeat sampling and Triggered source sampling following a routine TC+ sample. Contact DDW if not sure on follow up sampling.
2. Follow sanitary sampling protocol of sufficient flushing and disinfection, using a strong
chlorine-based solution (applying, allowing contact time, flushing, sample collection). 3. Inspect and Clean Storage Tank WS 001 on a regular basis, Disinfect as per AWWA C651 standards as an O& M procedure if possible.
