In accordance with Hill Air Force Base Title V Operating Permit 300036005, condition I.S.2.c
and Utah Administrative Code (UAC) R-307-415-6a, I need to notify you of a deviation from condition
11.B.4.d.3 pertaining to "Internal Combustion Engines" that states: "The permittee shall submit the
following notifications. Notification of new or modified equipment installation must be submitted to
Utah Division of Air Quality seven days prior to such installation." A second deviation from I.S.2.c "The
Director shall be notified promptly of any deviations from permit requirements" resulted as it was not
discovered until after the 21-day prompt period had passed.
In July 2022, our organization temporarily replaced a 100-Kilowatt (KW) emergency generator
with a portable 150 KW generator but failed to submit a seven-day flexibility notification prior to
installation of a generator larger than the existing permitted source. Once discovered, we corrected the
issue and replaced the 150 KW generator with a 100 KW generator. We also retrained the personnel
involved to ensure we meet all permitting requirements of a stationary source even when dealing with
portable equipment only intended for temporary replacement of an existing stationary source.
In accordance with UAC R307-415-5d and based on information and belief formed after
reasonable inquiry, I certify that the statements and information in this document are true, accurate and
complete.
