In accordance with Operating Permit provision I.S.2.a, and Utah Administrative Code (UAC) R307415-
6a(3)(c)(i), the following monitoring report is submiued.
This report covers the period from May 1,2023 to October 31,2023.
For the indicated reporting period:
There were no deviations from any permit requirements.
Attached is a list of permit requiranents for which thoe were deviations, The list
includes the information required by Operating Permit provision I.S.2.a, and
UAC R3 07-a I 5-6a(3XcXi).
In lieu of attaching the above list, deviation reports submitted in accordance witr
permit provision I. S. 2.c, and UAC R3 07 -4 I 5 6a(3 Xc)(ii) are referenced below.
