Dear Lester Dalton: The Division of Drinking Water (the Division) received the following documents on October 30, 2024: The water line installation drawings and specifications for Washington City. These were previously submitted in 2018, and no changes have been made and still meet the minimum requirements. The completed application form signed by Lester Dalton and dated October 30, 2024, which formally designates: Dustyn Shaffer, P.E., as the Professional Engineer (PE) having direct responsibility for the design of the entire water system, and Nathan Wallentine, P.E., as the PE responsible for oversight of the hydraulic analysis of the entire water system. We completed our review of the water line installation drawings and specifications, and found they basically comply with the applicable portions of Utah's Rules for Public Drinking Water Systems in R309-550. On this basis, the Washington City water line installation drawings and specificationsare hereby approved. Washington City has met the criteria in R309-500-7 to install water lines up to 16 inches in diameter and is not required to obtain Plan Approval or an Operating Permit for the qualified water line projects. Please note the following restrictions to this approval: 1. This approval does not apply to booster pump stations. 2. If minimum separation between water and sewer lines cannot meet the requirements in R309-550-8, an exception must be obtained prior to water line construction. This Approval expires on December 31, 2029. You must notify the Division if there is a change to standard installation drawings and specifications or the designated PE for design or hydraulic analysis during the term of this Approval. To continue using standard installation drawings and specifications for water transmission and distribution lines after the expiration date, you must submit a complete renewal application to the Division. Please maintain a copy of this letter with your permanent records for future reference. If you have any questions regarding this approval, please contact J. Paul Wright, P.E., DEQ Southwest District Office at (435) 986-2590, or me at (385) 515-1464. Sincerely, Michael Newberry, P.E.
