Subject:Operating Permit, Lower Reservoir Recoating Project (ST002);

Dear Tracy Feltner:The Division of Drinking Water (the Division) received a request for an Operating Permit for the proposed Lower Reservoir Recoating Project from Curtis L. Nielsen,
Ensign Engineering, on October 31, 2024. Previously, the Division received the specifications from you on August 2, 2024.The Division issued Plan Approval for this project on August
20, 2024.
Our understanding of the project was that NW Tank Lining and Inspection, Inc. (NWTLI) recoated the interior of Lower Reservoir (ST002). They cleaned the tank and abrasively blasted the
tank again before applying an epoxy coating. The coating that was applied is Sherwin Williams MACROPOXY 5500LT POTABLE WATER EPOXY Certified to NSF/ANSI 61 standards. Once applied, the
product was cured to its specifications. After sufficient cure time was allowed, the coating was inspected for proper coverage and quality. Water operators disinfected the tank, flushed
the tank, and Bacteriologic and VOC samples were taken.
We have received the following information for the Lower Reservoir Recoating Project:
Certification of Rule Conformance with plan approval conditions by a professional engineer.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
VOC samples were submitted.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Lower Reservoir Recoating Projectis hereby issued as constituted
by this letter. You may now place this tank in service in your water systemIncreased VOC MonitoringThe October 23, 2024, VOC sample collected from the Lower Reservoir tank yielded detections
for Ethyl Benzene, Toluene and Xylenes, total at 10.5 ug/L 0.7 ug/L and 52.7 ug/L respectively. Although these are below respective maximum contaminant levels these results require increased
VOC monitoring.A VOC sample collected from the outflow of the Lower Reservoir tank is now required each calendar quarter. The next VOC sample is due sometime between January 1, 2025,
and March 31, 2025. This requirement will be reevaluated once two consecutive quarters VOC samples have been gathered. Please contact David Kruse at (385) 566-7789 or dbkruse@utah.gov
