Howard Director June 5, 2019 Kathy Weinel, Quality Assurance Manager Energy Fuels Resources (USA) Inc. 225 Union Blvd., Suite 600 Lakewood, CO 80228 RE: Energy Fuels Resources, White Mesa Uranium Mill, Groundwater Quality Assurance Plan Version 7.5 Utah Groundwater Discharge Permit No. UGW370004 Dear Ms. Weinel: The Utah Division of Waste Management and Radiation Control (Division) has completed review of the Energy Fuels Resources (USA) Inc. (EFR) May 14, 2019 “Revision of Groundwater Quality Assurance Plan (QAP).” Per review the submittal includes a redline strikeout version and a clean copy of the proposed QAP Version 7.5. The following changes from the previously approved version were noted: 1. Headers were revised to list the current QAP version and date. 2. The Table of Contents was updated and typographical errors were corrected throughout the document. 3. Chloroform pumping wells TW4-40 and TW4-41 were added to the groundwater head monitoring frequency requirements (Section 6.1.2, Page 13). 4. Dissolved oxygen was added to the requirements for field parameters to be collected with quarterly and semi-annual sampling (Section 6.2.2, Page 14). 5. Dissolved oxygen was added to the list of field data information (Section 7.1, Page 17). 6. The quarterly depth to water data sheet was restructured/columns relabeled (Page 35). 7. Dissolved oxygen was added to the purging procedures parameter list (Attachment 2-3, Page 44). 8. Dissolved oxygen was added to the procedures for field measurement when two casing volumes and three casing volumes are purged (Attachment 2-3, Pages 46, 47). 9. The chloroform investigation monitoring wells list was updated (Chloroform QAP, Page 53). Based on Division’s review the changes appear to be appropriate and the QAP Version 7.5 is hereby approved. Please refer to and ensure that the requirements of this plan are followed for future groundwater sampling activities and reports.
