RE: August 15, 2017 Transmittal of Revised Groundwater Quality Assurance Plan
Utah Groundwater Discharge Permit No. UGW370004
Dear Ms. Weinel:
The Division of Waste Management and Radiation Control has completed its review of the Revised
Groundwater Quality Assurance Plan (QAP). The transmittal includes a strike-out copy of the
previously approved QAP (Rev. 7.2), showing the revisions made and a copy of the final QAP, Revision
7.3.
A primary change made to the QAP was the inclusion of a new electronic data collection process and
reporting procedures. Based on discussions with the Division and EFR, the new reporting format will
include all information currently required by Rev. 7.2, will be cross checked for accuracy and will be
officially submitted to Division via hardcopy data collection forms.
Other changes to the QAP Rev. 7.3 included updating the company ownership name, Division name,
well identification names and eliminating references to a specific individual laboratory. Several new
wells and identification names were updated.

It was also noted that the QAP Appendices A and B relating to the chloroform and nitrate plume
corrective action monitoring were updated to reflect currently installed and monitored well locations and identification names. Specific reporting requirements for the chloroform project were removed and
replaced with a reference to the Groundwater Corrective Action Plan (GCAP). The GCAP provides a
detailed description of quarterly monitoring and reporting requirements and the QAP Rev. 7.3 change
appears to be appropriate.
Based on Division’s review, the QAP changes are appropriate and the August 15, 2017 QAP, Rev. 7.3 is
hereby approved.
If you have any questions, please call Tom Rushing at (801) 536-0080.
Sincerely,
Scott T. Anderson, Director Division of Waste Management and Radiation Control
