Dear Ms. Weinel:
The Division of Waste Management and Radiation Control has completed its review of the 2016 Annual
Seep and Springs Sampling Report, dated November 17, 2016. The purpose of the review was to
evaluate Energy Fuels’ compliance with the following:

1. White Mesa Uranium Mill Ground Water Quality Assurance Plan (QAP). Revision 7.2, dated June 6, 2012.
2. Groundwater Discharge Permit No. UGW370004 (Permit). Permit version that was signed on
August 24, 2012.
3. Sampling Plan for Seeps and Springs in the Vicinity of the White Mesa Uranium Mill, Revision
0 (Plan).
The Division concluded that the Report is in compliance with the Permit, QAP and Plan. This letter
constitutes a closeout of the 2016 Annual Seep and Springs Sampling Report.
