Dear Steve Mcintosh:

On September 2, 2021, the Director of the Division of Drinking Water (“Director”) issued a
Compliance Agreement Enforcement Order (“the Order”) to the Mt Haven Owners Association.
As authorized in Number 11 of the General Provisions section of that Order, the Director is
amending that order. The original order remains a final order. The Director’s amendments are
attached in the Supplemental Enforcement Order Schedule (“EOS”). Please sign and return the
attached Supplemental EOS within 30 days.

The System will remain rated “Corrective Action.” Please continue working with Division staff
and your local health department to remain compliant.

Under Utah law, the Director of the Division must provide notice of violations of the Utah Safe
Drinking Water Act (the “Act”), the rules, or orders and enforce compliance with regulatory
requirements otherwise. This includes violations of the Order, which remains a final
administrative order.
