White Mesa Uranium Mill, near Blanding, Utah On August 13, 2013 the Utah Division of Radiation Control ("DRC") staff Dean Henderson and Phil Goble observed Energy Fuels Resources (USA), Inc. ("EFR") staff Tanner Holiday and Garrin Palmer perform the monitoring and sampling for the 2013 annual tailings cells, leak detection systems and slimes drams waste water sampling event. The DRC collected a split waste water sample from tailing cell 1. A duplicate sample of tailing cell 1 was also collected and is identified as Cell 6. On August 14,2013 samples being analyzed for radiologics were shipped UPS ground to Eberline Services Laboratory and samples being analyzed for all non-radidlogics was shipped next day air by FedEx. A written review on the DRC sample results and companson with the EFR sample results along with a review of EFR compliance with the their Sampling and Analysis Plan for Tailings Cells, Leak Detection Systems and Slimes Drains ("Sample Plan") will be completed after the 2013 Annual Tailings Cells Wastewater Sampling Report is submitted to DRC on December 1,2013. The DRC staff also observed the EFR staff collect waste water samples from tail ng Cell 1, Cell 2 slimes drain (see figures 1 and 2), Cell 3 (see figures 3 and 4), Cell 4A (see figures 5 and 6), Cell 4A leak detection system ("LDS") [see figure 9], and Cell 4B (see figures 7 and 8). Fluid levels in the LDS monitors for Cell 1 (0.4"), Cell 2 and Cell 3 (0.0'), and Cell 4A (-0.29 to 0.55") indicated that there was not enough fluid to sample therefore, no samples were collected from these LDS. Samples for Cell 1, Cell 3, Cell 4A, Cell 4A LDS, and Cell 4B were collected with the use of a disposable ladle. The sample for Cell 2 slimes drain was collected using a disposable bailer (see figures 1 and 2). After observing the EFR staff perform the sampling and monitormg it appears they appropriately followed QAP and Sample Plan. A closeout meeting was held with Mr. Dan Hillsten following the completion of sampling activities.
