Groundwater Discharge Permit No UGW370004 ("Permit") Permit version that was signed on
July 14, 2012
Sampling Plan for Seeps and Spnngs In the Vicinity of the White Mesa Uranium Mill Revision 0
("Plan")
The review found that Energy Fuels Resources (USA), Inc Report appeared to be in compliance with the
Permit, QAP, and Plan This letter constitutes a closeout of the 2012 Annual Seep and Spnngs Sampling
