OF T///*t, State of Utah GARY R HERBERT Governor GREG BELL Lieutenant Governor Department of Environmental Quality Amanda Sinith Executive Director DIVISION OF RADIATION CONTROL Rusty Lundberg Director 0-2012-001965 MEMORANDUM TO FROM DATE SUBJECT Phil Goble Pl^ Dean Henderson September 10, 2012' 0^ 2012 Annual Seep and Spnngs Sampling and Utah Division of Radiation Split sampling Energy Fuels Resources (USA), Inc White Mesa Uranium Mill, near Blanding, Utah On June 20, 2012 the Utah Division of Radiation Control (hereafter "DRC") staff Dean Henderson and Phil Goble observed Energy Fuels Resources (USA), Inc (hereafter "EFR") staff Tanner Holiday and Gamn Palmer perform thr monitonng and sampling for the annul 2012 seep and spnngs monitoring event The DRC collected a split sample of Cottonwood Spnng (see photograph in Figure 1) DRC also collected a duplicate sample of Cottonwood Spnng and is identified as Spnng 1 On July 22, 2012 both samples were shipped to Lion-viUe Laboratory by UPS in two coolers One cooler was shipped next day air that earned portions of the samples in containers packed m ice to be analyzed for the non-radiologics listed in Table 1 of Section 8 2 of EFR Ground Water Monitonng Quality Assurance Plan (hereafter "QAP") The other cooler was shipped by ground that had portions of the samples in containers to be analyzed for gross alpha A report on DRC sample results and companson with EFR sample result will be completed after the 2012 seeps and springs monitonng report is submitted of DRC on December 1,2012 The DRC staff also observed the EFR staff sample Entrance (see photograph m Figure 3) and Ruin Spnngs (see photograph in Figure 2) Westwater Seep had a seepage face, but had no standing water so therefore no sample could be collected (see photograph in Figure 4) Coral Canyon and Corral seeps were dry and therefore no sample could be collected After observing the EFR staff perform the sampling and monitonng it appears they appropnately followed the QAP and the Sampling Plan for Seeps and Spnngs A closeout meeting was held with Mr Dan Hillsten following the completion of sampling activities Mr Hillsten was told that the sampling acti-vaties appeared to go smoothly with no apparent problems He was also reminded to continue to check on Westwater Seep in case there is found to be enough water to collect a sample 195 North 1950 West • Salt Uke City, UT Maihng Address P O Box 144850 • Salt Lake City, UT 84114-4850 Telephone (801) 536-4250 • Fax (801) 533-4097 'TDD (801) 536^14 www deq Utah gov Pnnted on 100% recycled paper Page 2 References Denison Mines (USA) Corp , March 2009, Sampling Plan for Seeps and Spnngs In the Vicinity of the White Mesa Uranium Mill Re-vision 0 Demson Mines (USA) Corp , February 22, 2010, White Mesa Uranium Mill Ground Water Quality Assurance Plan Revision 5 0 Page 3 Attachment 1 Figures 1 through 4 Figure 1.
