The Utah Division of Radiation Control has completed the review of 3''' Quarter 2011
Chloroform Monitonng Reports (hereafter Report ) for the Chloroform Contamination
Investigation

The review was based on compliance with the Ground Water Quality Discharge Permit Number
UGW370004 (hereafter Permit ) and the DUSA Quality Assurance Plan (hereafter QAP )
Dunng the DRC review it was found that I)USA failed to meet the requirements found in Section
6(c)(3rd bullet) of Appendix A of the QAP This section of the QAP requires
Calculate chloroform mass removal by pumping wells Calculations would include 1) total
historic chloroform mass removed 2) total historic chloroform mass removed for each pumping
well 3) total chloroform mass removed for the quarter and 4) total chloroform mass removed
from each pumping well for the quarter i
DUSA reported incorrect total mass removal for the 3 Quarter 2011 for pumping wells MW 24
TW4 4 TW4 19 and T-W4 20 and the total mass remoyal for the quarter Using volumes
(gallons) pumped and chloroform concentrations reported for each pumping well m the Report
the Executive Secretary calculated 8 1 lbs of chloroform removed for the quarter and DUSA
reported (calculated) 13 2 lbs of chloroform removed for the quarter It appears that DUSA

mistakenly used the 2nd quarter 2011 chloroform concentrations for pumping wells MW 24 TW4

Notice of Enforcement Discretion
The Executive Secretary has decided to use enforcement discieUon in this matter Please ensure
that future reports will include accurate chloroform mass removal data
The remainder of the items reviewed were found to be conducted in compliance with the Permit
and QAP This letter consututes a closeout of the 3''' Quarter 2011 Chloroform Monitonng
Reports for the Chloroform ContammaUon InvesUgation
