SUBJECT: August 9-10, 2011 DUSA 2011 BAT/DMT Inspection;
Engineering Inspection Module 75E; Findings and Closeout
An inspection on the subject was held on August 9-10, 2011 at the DUSA mill facilities near Blanding,
Utah. Mr. David Rupp represented DRC, Messrs. Ryan Palmer and Daniel Hillsten represented DUSA. A
brief discussion of the references and findings of the inspection follow below:
Findings and Discussion:
No new problems were identified. All facilities appear to be functioning in accordance with the current
DMT/BAT requirements, except as separately identified in current DRC documented actions.
Therefore, the 2011 DMT/BAT Inspection Module 75E is hereby closed-out. Please contact Mr. David
Rupp of DRC, if you have any questions on the above.

Findings and Closeout
An inspection on the subject was held on August 9-10, 2011 at the DUSA mill facilities near Blanding,
Utah. Mr. David Rupp represented DRC, Messrs. Ryan Palmer and Daniel Hillsten represented DUSA. A
brief discussion of the references and findings of the inspection follow Below:

Findings and Discussion:
No new problems were identified. All facilities appear to be functioning in accordance with the current
DMT/BAT requirements, except as identified in current DRC documentation.
The captioned photographs in reference (A) show acceptable events occurring at the tailings cells and mill
area. The record of the interview in reference (B) was obtained during the inspection. However, the
interview information has been included with the documentation of two other separate DRC actions. One
action involves non-measurement of the water level in Cell 4B (sfill under DRC review). The other action
has been resolved by an NOED dated September 22, 2011, and involved data logging problems for the
LDS in Cell 4A. Both these events are documented in DRC files.
