Box 144810 Salt Lake City, UT 84114-4810 C-201 Re: State of Utah Ground Water Discharge Permit No. UGW370004 White Mesa Uranium Mill - Notice of Plan to Install Weil Pumps Dear Mr. Lundberg: By this letter, Denison Mines (USA) Corp. requests the Executive Secretary's approval for the installation of dedicated pumps in three monitoring wells at the White Mesa Mill as described below. New groundwater monitoring wells MW33, MW34, and MW35 were installed on the dike of Cell 4B on September 2, 2010, as required by Part I.H.4 of the Mill's Groundwater Discharge Permit ("GWDP") dated June 17, 2010. Because MW33 was determined to be a dry well and MW34 produced insufficient water for sampling, DRC revised the GWDP on February 15, 2011, in part, to address these hydrogeologic findings. As a result, the February 15, 2011 GWDP removed requirements to sample MW33 and MW 34. (However groundwater levels are monitored in MW34). New groundwater monitoring wells MW36 and MW 37 were subsequently installed on the dike of Cell 4B on April 26 and 25, 2011, respectively, to replace MW33 and MW34 as monitoring wells. MW-35 has been sampled during the third and fourth quarter 2010 and first quarter of 2011 by means of a portable pump. MW36 and MW37 are undergoing initial development and have not yet been sampled. To be consistent with the rest of the groundwater monitoring program (all other point of compliance wells are sampled using dedicated pumps), and to minimize the opportunity for cross-contamination from re-use of a portable pump, the Mill wishes to install dedicated pumps within the screened intervals of each of these three wells as described below. The proposed pumps are of identical type and size as dedicated pumps in all other wells in the Mill's groundwater monitoring program. The As-Built Report for MW36 and MW37 will be submitted on or before June 30, 2011. Part I.E.11 of the Mill's GWDP requires that "before any modification of groundwater monitoring or analysis procedures, methods of equipment, the Permittee must obtain prior written approval from the Executive Secretary." By this letter, Denison is requesting the Executive Secretary's approval for installation of the above-listed pumps. Denison will continue to sample all of the above named wells by means of a bailer until we receive DRC approval for pump installation.
