White Mesa Uranium Mill Supplemental Chloroform Monitoring Report Addendum State of Utah Notice of Violation and Groundwater Corrective Action Order IJDEQ Docket No UGQ-20-O1 3rd Quarter July through September 2008 Prepared by Denison Mines USA Corp DUSA December 2009 Introduction The requirements of the White Mesa Uranium Mill Groundwater Monitoring Plan Quality Assurance Plan QAP were modified and approved as Revision to the QAP on June 20 2009 With this revision Appendix of the QAP Chloroform Investigation Monitoring Quality Assurance Plan was modified to include additional reporting requirements for Quarterly Chloroform Investigation Monitoring Reports Contrary to these requirements and by inadvertent oversight these new reporting needs were not addressed by the 3M Quarter 2008 Chloroform Monitoring Report In discussions with UDEQ representatives on October 29 2009 and in response to UDEQs findings dated November 10 2009 Denison proposed to amend the report by means of preparing supplement to the report Accordingly this supplement to the 3rd Quarter 2008 Report was prepared in order to amend the Report by inclusion of the following information reporting of the historic mass removal of chloroform from the shallow aquifer to date reporting of total historic chloroform mass removed from each pumping well to date reporting of chloroform mass removed from each pumping well in the quarter reporting of the total chloroform removed in the quarter revised isoeoneentration map to display the 70 ug/l isoeoneentration contour line and listing of the historic analytical results which includes the results of analyses for carbon tetraehloride chloride chloromethane and methylene chloride In addition to these data information pertaining to monitoring wells TW4-23 TW4-24 and TW4-25 with regard to the listing of historic groundwater elevation data and the inclusion of hydrographs to display groundwater elevation data overtime were not provided in the 3rd Quarter 2008 report Accordingly Denison also agreed in the October 29 2009 discussion with UDEQ and in response to the November 10 2009 UDEQ correspondence that the 3rd Quarter 2008 would be updated with respect to these wells by means of supplemental amendment of the report In accordance with these additional information needs this supplemental addendum to the report was prepared in order to provide the following supplemental information pertaining to the 3rd Quarter 2008 Quarterly Chloroform Monitoring Report chloroform mass removal information is updated by this supplement through the creation of new section of the report Section 4.4.5 Mass Removal the graphic display of water level elevations is supplemented by providing additional hydrographs for wells TW4-23 TW4-24 and TW4-25 pertaining to Tab of the Report the listing of water elevation data is updated to include listings for wells TW4-23 TW4- 24 and TW4-25 pertaining to at Tab of the Repurt the isoeoneentration map has been modified to display the 70 ug/l concentration contour line pertaining to Tab of the Report and historical data relating to analyses for carbon tetraehloride chloride ehloromethane and methylene
