DMT PERFORMANCE STANDARD MONITORING REPORT
1.0 Mt^asurements Taken During the Quarter
During the Quarter, thc folluvving DMT Monitoring was performed nr adclrcsscd, as
required under Pan. T.E.6 of the Permit:
-Weekly tailings wastewater pool elevations for tailings (.Iclls 1,3 and 4A;
-Monthly slimes drain water levels;
-Weekly wastewater level measurements m Robcrt'.s Pond; atid
-Weekly feedstock storage area inspections.
2.0 Weekly Tailings Waste Water Pool Elevation Monitorins
Mill personnel monitored and recorded weekly the elevation of wastewater in Tailings Cells 1, 3
and 4A lo ensure compliance with the maximum wastewater elevation criteria mandated by
Condition 10.3 of the Mill's Radioaclive Materials License. These measurements were made
from an elevation survey. The results of such monitoring, reported as feet above me^Hu sea level
(frnsl), arc included in Attachment A,
Condition 10.3 ofthe Mill's Radioactive Materials License provides that freeboard limits for
Cells 1 and 3 shall be set in accordance with procedures thai have been approved by thc U.S.
Nuclear Regulatory Commission. Llnder those procedures:
(i) The freeboard limits are set as per thc January 10. 1990 Drainage Report for Cell
I at a liquid maximum elevation of .5,615.4 fmsl; and
(ii) The freeboard limit for Cell 3 is determined armually using a toimula scl out in
the procedure. The current freeboard liinit for Cell 3 was previously calculated
under this procedure at 5,601.6 fmsl. However, in UDEQ correspondence dated
November 20, 2008 an interim variance and liniii was established at 5,602.5 fmsl.
(iii) In conjunction with the variance established under (ii) above, an interim
maximum elevation for Cel 14A was also established at 5,593.74
In addition. Part LD.2 of the Permit provides that under no circumstances shall die freeboard of
any tailings cell bc less than three feel, a.s measured from the top of the flexible membrane liner
("FML"). The top of the FML in Cell 1 is at 5,618.5 fmsl and lhe top of the FML in Cell 3 is at
5,608.5 fmsl. This means thai the maximum wastewater pool elevations in Cells 1 and 3
permitted under Pan LD.2 of the Permit are 5,615.5 and 5,605.5 fmsl, respectively.
As indicated in lhe table below, thc wasle water pool elevations in Cells 1 and 3, as measured
during thc Quarter, did not exceed these regulatory .standards:

Note: Interim ma.ximum clevaiions of 5.602,5 fmsl and 5,593.74 fmsl for Cells 3 and 4A,
respectively have been established in correspondence from UDEQ dated November 20, 2008. Thc
maximum elevation of 5,593.74 fmsl for Cell 4A has been set assuming diat the total PMP volume
for Cells 2, 3 and 4A will be accommodated in Cell 4A. By letter dated December 11, 2008 Denison
applied for an amendment to ils Radioactive Materials License to set tlie freeboard limit for Cell 4A
al 5,593.74 fmsl and to eliminate the need to set a freeboard limit for Cell 3, given that the freeboard
limit of 5,593.74 fmsl for Cell 4A is .idcLjuate to accommodate the total PMP volume for CelLs 2. 3
and 4A,
3.0 Monthly Slimes Drain Waler Level Monitoring
Pursuant to Patt 1.F.2 of the Groundwater Discharge Permit and correspondence from the UDEQ
dated February 7, 2008, the rcsulls of monthly recovery monitoring of the slimes drain for Cell 2 are
to be recorded and included with the resulrs of cjuarlerly monitoring in the Quarterly Report. In
accordance with these requiremenis, the monthly readings and a graph showing the initial reading
and thc final reading for each monlh are included as Attachment B. These readings are to be
collected during a 90 hour recovery period aftei' the slimes drain pump has been turned off iri order to
delennine the static water level in die drain (head) after this short cessation in pumping.
In Part l.E.6.b) of tlie Permit this requirement shall apply to Cell 3 only upon initiation of tailings de-
watering operations. As Tailings Cell 3 is currently an active tailings cell and de-watering operations
have not commenced, no such measurements will be made in lhe slimes drain access pipe for Cell 3
in the foreseeable future.
4.0 Weekly Wasiewater Level Monitoring: Roberts Pond
Mill personnel monitored and recorded weekly, commencing on March 8, 2005, the effective date of
thc Permit, the wastewater levels at Roberts Pond to detemiine compliance with the DMT operations
standards in Pari LD.3. uf the Pennil. Fait I .D.3 of the Pennit provides that the water level in
Robcn's Pond shall not exceed an elevation of 5624 fmsl. Thc results of such monitoring, reported
as feet above mean sea level (fmsl),

The maximum wastewater elevation measured in Robcii's Pond during the quarter was 5620:56
fmsl which did not exceed the maximum permitted elevation of 5,624 fmsl.
5.0 Weekly Feedstock Storage Area Inspection
Weekly feedstock storage area inspections wcrc performed by the Radiation Safety Department,
coimncncing on March 8, 2005, the effective dale of the Pentiit, to confirm that the bulk
feedstock materials are stored and mainuiined within the defined area described in the Permit and
that all alternate feedstock localed outside the defined feedstock area arc maintained within water
light containers. The results of these inspections are recorded on thc Weekly Mill Inspection
Forms, which are available at the Mill for inspection. No variance in stored materials from this
requirement or leaking alternate feedstock drums or other alternate feedstock containers were
observed during the inspections.
