Shelley Executive Director DIVISION OF DRINKING WATER Nathan Lunstad, Ph.D., P.E. Director October 1, 2024 Zachary Renstrom Washington County WCD – Sand Hollow 533 Waterorks Dr St George, UT 84770 Subject: Notice of Source Deactivation for Washington County WCD – Sand Hollow; UTAH27073 Zachary Renstrom: This letter serves as notification that the Division of Drinking Water (the Division) has changed the activity status of the following sources from “Active” to “Inactive”: • Sand Hollow Well 9 (identified as WS003) The listed sources are now considered inactive and are not allowed to be utilized as a source of drinking water until they have been formally reactivated. This decision was made after Brandon Robinson notified the Division on September 25, 2024, that the Sand Hollow Well 9 (WS003) had been physically disconnected from the culinary system and is now used solely in the secondary water system. The Sand Hollow Well 9 (WS003) no longer has monitoring requirements. However, since the Sand Hollow Well 9 (WS003) is not designated as abandoned it will continue to be inspected during triennial sanitary surveys. The Sand Hollow Well 9 (WS003) may be designated as abandoned from the culinary water system once the Division has received either pictures showing a physical disconnection or engineering plans documenting the project in which the well was disconnected. Please contact David Kruse at dbkruse@utah.gov or 385-566-7789 to have the Sand Hollow Well 9 (WS003) designated as abandoned from the culinary water system. Thank you for your efforts in maintaining a safe drinking water system. Zachary Renstrom Page 2 of 2 October 1, 2024 195 North 1950 West • Salt Lake City, UT Mailing Address: P.O.
