PROCESS DESCRIPTION: ImageFIRST Healthcare Laundry Specialists operates a non-greasy non-biohazardous textile laundry located in Salt Lake City. Sheets and clothing articles from hospitals are washed and sterilized prior to being shipped back to the customers. APPLICABLE REGULATIONS: Approval Order (AO) DAQE-AN122550004-22, dated February 25, 2022 NSPS (Part 60) Dc: Standards of Performance for Small Industrial-Commercial-Institutional Steam Generating Units, SOURCE EVALUATION: Name of Permittee: Permitted Location: ImageFIRST - Healthcare Laundry Specialists 900 East Eighth Ave Suite 200 2295 Custer Road King of Prussia, PA 19406 Salt Lake City, UT 84104 SIC Code: 7211: (Power Laundries, Family & Commercial) Section I: GENERAL PROVISIONS I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the Utah Administrative Code (UAC) Rule 307 (R307) and Title 40 of the Code of Federal Regulations (40 CFR). Unless noted otherwise, references cited in these AO conditions refer to those rules. [R307-101] I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401] 2 I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions covered by this AO must be reviewed and approved. [R307-401-1] I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by the owner/operator, shall be made available to the Director or Director's representative upon request, and the records shall include the two-year period prior to the date of the request. Unless otherwise specified in this AO or in other applicable state and federal rules, records shall be kept for a minimum of two (2) years. [R307-401-8] I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall, to the extent practicable, maintain and operate any equipment approved under this AO, including associated air pollution control equipment, in a manner consistent with good air pollution control practice for minimizing emissions. Determination of whether acceptable operating and maintenance procedures are being used will be based on information available to the Director which may include, but is not limited to, monitoring results, opacity observations, review of operating and maintenance procedures, and inspection of the source. All maintenance performed on equipment authorized by this AO shall be recorded. [R307-401-4] I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns. [R307-107] I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150] Status: Out of Compliance. No limits set forth in this AO appear to have been exceeded. An unapproved water heater was discovered following the inspection. See Section II for more details. The equipment appeared to be properly operated and maintained according to manufacturer recommendations. Records are kept as required and were made available after the inspection. No breakdowns have been reported since the previous inspection. This source is not required to submit an emissions report at this time. Section II: SPECIAL PROVISIONS II.A The approved installations shall consist of the following equipment: II.A.1 Textile Care Services Inc. Laundry Facility II.A.2 Two (2) Boilers - Cleaver Brooks Fuel: Natural Gas Heating Capacity: 16.8 MMBTU/hr each. Controls 48ppm NOx flue gas recirculation NSPS Applicability: Subpart Dc II.A.3 Various Dryers Fuel: Natural Gas Maximum Heating Capacity: less than 5.0 MMBTU/hr each. Status: Out of Compliance. An unpermitted water heater is on site with a rating of 6.3 MMBtu/hr. See the attached picture of the nameplate. A Compliance Advisory (DAQC-877-24) was issued on August 28, 2024. The source responded on September 4, 2024. Based on their response, a Warning (DAQC-949-24) was issued on October 16, 2024. The source is decommissioning this unpermitted water heater and replacing it with an electric steam water heater that does not need to be permitted on the AO. Construction of this replacement began on October 9, 2024. No further action is recommended at this time. The boilers have not been replaced and there are currently no plans to do so. All dryers are less than 5 MMBTu/hr. There are 6 Lavatec dryers and 7 Washex dryers on site. See the attached email correspondence for pictures of the manufacturer’s plates for these dryers. 3 II.B Requirements and Limitations II.B.1 Facility-Wide Requirement II.B.1.a At least once per calendar year, all employees who operate equipment (operator) that produces and/or controls emissions to the air shall receive proper training as to their responsibilities in operating that equipment according to all relevant conditions of this AO. The training for each operator shall be for all equipment that operator operates and the required training shall only be for pollution control equipment and/or procedures that have a direct effect on emissions to the ambient air. The equipment shall include all of the equipment listed above and any other equipment that affects or produces air emissions that the operator operates, this includes source category exempt equipment. Within 60 days of every time this AO is modified or reissued, those employees who operate equipment that produces and/or controls emissions to the air that is affected by the AO changes shall receive proper training as to their responsibilities in operating equipment according to all relevant conditions of this AO. Within 60 days of a new operator being employed or assigned with the job responsibility to operate any of the equipment that produces and/or controls emissions to the air, the new operator shall receive proper training as to their responsibilities in operating the equipment according to all relevant conditions of this AO. Records of operator training shall be made available to The Director upon request. This AO shall be made available to all employees who operate the equipment listed in this AO. [R307-401-8] Status: In Compliance. The source stated that their staff receives regular training to reduce emissions as possible and to operate equipment as recommended by the manufacturer. II.B.2 Limitations. II.B.2.a The owner/operator shall not exceed the following visible emissions from the following emission points: A. All boilers and dryers stacks - 10% opacity B. All other points - 20% opacity [R307-401-8] II.B.2.a.1 Opacity observations of emissions from stationary sources shall be conducted according to 40 CFR 60, Appendix A, Method 9. [R307-401-8] Status: In Compliance. No visible emissions were observed from any point at the time of inspection. Method 9 was utilized to verify opacity limits. See attached VEO form. II.B.2.b The owner/operator shall not consume more than 56.40 MMscf of natural gas per rolling 12-month period in all boilers and dryers combined. [R307-401-8] II.B.2.b.1 The owner/operator shall: A. Determine consumption of the two 16.8 MMBtu/hr boilers with a flow meter and consumption for all other dryers with a gas bill. B. Record consumption of the two 16.8 MMBtu/hr boilers on a daily basis and consumption of all other dryers on a monthly basis. C. Use the consumption data to calculate a new rolling 12-month total by the 20th day of each month using data from the previous 12 months. 4 D. Determine consumption by supervisor monitoring and maintaining of a consumption log. E. Keep the consumption records for all periods the plant is in operation [R307-401-8] Status: In Compliance. Consumption records appear to be calculated correctly. For the rolling 12-month period from March 2023 - February 2024, a total of 36.458 MMScf of natural gas was consumed. Since November 2023, the source has been able to separate total natural gas usage between the boilers and water heaters from that of the dryers. The source stated that they will provide this separation going forward. See the attached reports, spreadsheets, and email correspondence. II.B.2.c The owner/operator shall use only natural gas as a fuel in the boilers and dryers. [R307-401-8] Status: In Compliance. Only natural gas is used as fuel in the boilers and dryers. See the attached monthly reports from Dominion Energy. Section III: APPLICABLE FEDERAL REQUIREMENTS In addition to the requirements of this AO, all applicable provisions of the following federal programs have been found to apply to this installation. This AO in no way releases the owner or operator from any liability for compliance with all other applicable federal, state, and local regulations including UAC R307. NSPS (Part 60) Dc: Standards of Performance for Small Industrial-Commercial-Institutional Steam Generating Units Status: In Compliance. The applicable requirement associated with this subpart is to maintain records of natural gas use. This requirement is satisfied as all-natural gas consumed at the source location is provided by Dominion Energy, and the source maintains monthly records of all-natural gas use at the source location. AREA SOURCE RULES EVALUATION: The following Area Source Rules were evaluated during this inspection: Stationary Sources [R307-210] Status: In Compliance. Compliance with this area source rule is satisfied by compliance with Federal Requirements NSPS (Part 60) Subparts Dc. See Section III above for more information. Solvent Cleaning [R307-304] Status: Not Applicable. The source stated that they no longer have a parts washer at this facility. Nonattainment and Maintenance Areas for PM10: Emission Standards [R307-305] Status: In Compliance. See Condition II.B.2.a above for more information. 5 EMISSION INVENTORY: The emissions listed below are an estimate of the total potential emissions (PTE) from ImageFIRST - Healthcare Laundry Specialists on the Approval Order (AO) DAQE-AN122550004-22, dated February 25, 2022. PTE are supplied for supplemental purposes only. COMPLIANCE STATUS & RECOMMENDATIONS: In regards to Approval Order (AO) DAQE-AN122550004-22, dated February 25, 2022: Out of Compliance at the time of inspection. An unpermitted water heater is on site – however, this equipment is being decommissioned. See Section II for more details. A Compliance Advisory (DAQC-877-24) was issued on August 28, 2024. The source responded on September 4, 2024. Based on their response, a Warning (DAQC-949-24) was issued on October 16, 2024. No further action is recommended at this time. HPV STATUS: Not Applicable. RECOMMENDATION FOR NEXT INSPECTION: Inspect at typical frequency for this source. Check to see that all dryers are below 5 MMBtu/hr. When Rick Taft owned the plant in 2020, the plan was to replace the boilers. After the business was sold to Image FIRST, that plan was scrubed. We are still operation both 400HP Cleaver Brooks. When Image First took over they instituted sever different initiatives. One being a gas meter for the boiler separate from the gas dryers. We installed a gas meter reading the total usage from the boilers. There is also a gas meter downstream from both boilers for the water heater. We can take the total usage, and subtract the boiler numbers. We also can subtract the water heater numbers from the boiler numbers. We have been recording the numbers from the meters on a regular basis since about October of last year. We use 2 different brands of dryers. Washex, and Lavatec. Attached is the name plate sample off of one of each of the dryers. There is 6 Lavatec dryers and 7 washex dryers. I belive that is everything we talked about. The main gas meter is in MCF and the other 2, boiler and water heater are in SCF. In the next few months, that water heater will be decommissioned and swapper out for a Steam bundle water heater that will not use gas to heat. This new steam heater will not need the use of gas to heat water. We got notice from our contractor just this morning that this water heater will be decommissioned and replaced with a steam unit on September 28, 2024. What do we need to do at this point? Re-inspect AFTER this water heater is removed? Your next inspection would be in the federal 2024 -2025 fiscal year at the earliest, which begins on October 1, 2024. The new steam heater will not need to be permitted and will not require submission of a NOI. However, I'll need to discuss with my manager on how to wrap up this compliance action. This new steam heater will not need the use of gas to heat water. We got notice from our contractor just this morning that this water heater will be decommissioned and replaced with a steam unit on September 28, 2024. What do we need to do at this point? Re-inspect AFTER this water heater is removed?
