project conducted at the R & L Carriers terminal located at 4375 1385 South Salt Lake
City, Utah on September 5, 2024.

September 5, 2024
1730 hours: EnviroServe received a request from Mr. Ken Moser of Offen
Petroleum to respond to a diesel fuel release at the R & L Carriers terminal
located at 4375 1385 S in Salt Lake City, Utah.
1800 hours: The EnviroServe response team loaded equipment for transport.
1830 hours: The response team mobilized to the site.
1850 hours: The response team arrived onsite and surveyed the scope of the
project. The project manager estimated approximately 100 gallons of diesel fuel
released during the filling of a diesel fuel above ground storage tank and
impacted a concrete and asphalt area measuring approximately 100 feet long by
20 feet wide. The project manager noted that no storm drains, or pervious areas
were impacted. The project manager contacted Mr. Ken Moser to provide a
project update.
1910 hours: The response team applied granular absorbents to the diesel fuel
release concrete area near the diesel fuel tank and worked them in utilizing long-
handled tools.
2000 hours: EnviroServe personnel collected and containerized the diesel fuel-
impacted absorbents from the concrete area into a 30 -gallon steel drum. The
response team applied granular absorbents to the diesel fuel release asphalt
areas and worked them in utilizing long-handled tools.
2215 hours: The response team collected and containerized the spent
absorbents from the asphalt within the previously generated 30 -gallon drum and
an additional 30-gallon steel drum. An environmental technician closed, labeled,
and loaded the drums for transport and future disposal activities.
2300 hours: The response team demobilized from the site returning to
EnviroServe's Salt Lake City Operations Center.
2400 hours: The response team arrived at EnviroServe's Salt Lake City
Operations Center. EnviroServe personnel restocked and resupplied while the
project manager began work on project documentation. All project -related
activities for the day were completed.

EnviroServe transported approximately 60 pounds of spent absorbent material
within two (2) 30-gallon steel drums to the Sump and Trap Cleaning facility in Salt
Lake City, Utah for disposal activities.
