Rule: R309-105-12(2)Rule requires each public water system to have a cross connection control program
that includes an operator with adequate training in the area of cross connection control
or backflow prevention.
Notes:Existing deficiency has not been corrected. The system's operator is schedule to take the
exam later this month.

and frame. This significant deficiency must be corrected within 120 days of notification

Notes:The portion of the weld on the access hatch is not sealed. Recommend sealing the gap.

continuously monitor turbidity in the filtered water. This significant deficiency must be
corrected within 120 days of notification or have a corrective action plan approved by
DDW.
Notes:Turbidity is not currently continuously monitored. System has ordered and will install the
necessary instrumentation.
