Dear Director,
In accordance with Hill Air Force Base Title V Operating Permit I100007004, condition
LS.2.c and Utah Administrative Code (UAC) R-307-415-6a,1need to notify you of a deviation from
II.B.4l .c for installing and filling an above ground diesel fuel tank prior to submitting a 7 -day
notification to the Utah Division of Air Quality (UDAQ).
On 12 October 2023, an independent contractor filled a 4,572 gallon above ground diesel fuel
tank with diesel fuel without the requiredT-day notification to UDAQ. We subsequently prepared
and submitted the 7 -day flex notice to UDAQ on I 7 October 2023 . Our environmental section also
trained the contractor personnel involved to ensure they fully understand the requirements associated
with the tank.
In accordance with UAC R307-415-5d and based on information and belief formed after
reasonable inquiry, I certiff that the statements and information in this document are true, accurate,
and complete.
