In accordance with HillAir Force Base Title V Operating Permit 1100007004, condition LS.2.c
and Utah Administrative Code (UAC) R307-415-6a, I need to notify you of a deviation from Il.B.53.g for
starting new boilers without submitting an initial notification within 30 days of the actualdate of start-up.
An additional deviation frorn condition LS.2.c occurred as the Director must be notified promptly of any
deviations from permit requirements.
Hill AFB has a newly constructed, unoccupied building ( l53l) that has two new 2.5 MMBtU/hr
boilers subject to Title 40 of the Code of Federal Regulations Part 63 Subpart DDDDD. On l2 June
2024, it was discovered that the new boilers were started to provide heat for the 202312024 season. The
contractor was unaware that their use during the ongoing construction of the building constituted the need
to submit an initial notification of actual start up prior to transferring control of the facility over to the
government. The contractor has been made aware of the requirement. Hill AFB submitted the initial
notification on I 7 June 2024 to correct this deficiency'
