In accordance with Hill Air Force Base Title V Operating Permit I100007004, condition I.S.2.c
andUtahAdministrativeCode(UAC)R-307-415-6a,lneedtonoti$,youofadeviation fromll.B.47.a.2
for failing to complete the required annual maintenance activities including the oil change in an existing
I l0 KW compressed ignition reciprocating internal combustion engine (RICE) subject to Title 40 Code of
Federal Regulations Part 63 Subpart ZZZZ. An additional deviation from condition I.S.2.c occurred as
the Director must receive prompt notification of any deviations from permit requirements.
During our annual base-wide audits in July, our environmental inspectors discovered maintenance
tasks for the RICE asset listed above did not get completed in April 2024 as scheduled. Once discovered,
the 309th Missile Maintenance Group (MMXG) competed the maintenance to include changing the air
cleaner and oil plus inspecting the hoses and belts. The 309 MMXG also identified the root cause as a
result of recent turnover in maintenance personnel. The organization trained the new personnel on the
required maintenance intervals and their supervisors will ensure the scheduled tasks get assigned
effectively. In total, the RICE asset only operated 2.1 hours after the oil change and inspections went past
due.
