Dear Ms. Chavez:

Rockwell Solutions completed well abandonment activities at the above-referenced site in
September and October 2024. Most of the remediation and many monitoring wells were
previously abandoned.

On August 27-28, 2024, the following 2-inch monitoring wells were abandoned:

MW-4
MW-5
MW-7
MW-8
MW-9
MW-11
MW-13
MW-14
MW-15
MW-16
MW-17
MW-18
MW-19
RW-12
RW-15

Also, the four-inch well RW-9 was abandoned.

Monitoring wells MW-10 and MW-12 could not be located and are assumed to have been
destroyed.

All wells were plugged/abandoned in accordance with current environmental industry standards
and Utah Division of Water Rights requirements. Each well was abandoned by Rockwell
Solutions personnel. The well can was removed, the well was filled with bentonite chips and cut
off below grade, and the surface was backfilled and patched to match the surrounding pavement

On October 14-15, 2024, injection wells IP-18 - IP-48, located in North Villa Drive were
plugged with bentonite. In agreement with a permit from Clearfield City, a two foot by three
foot area around the injection well was saw cut. The asphalt was removed, the hole was
backfilled and compacted, and the surface area was repaved with asphalt.

Figure 1 shows the locations of all of the site monitoring wells and the injection wells that were
abandoned. All monitoring wells and remediation wells for the site have been properly
abandoned.
