Salt Lake City, Utah Amended Additional Investigation Work Plan Basin Markets #105/Murray October 17, 2024 Page 1 1.0 INTRODUCTION Broadbent & Associates, Inc. is pleased to submit this Amended Additional Site Investigation Work Plan (Work Plan) which was prepared to assist with evaluating residual subsurface impacts of hydrocarbon mass and address data gaps associated with the Basin Markets #105 (Former Sinclair #43005) site located at 5380 South 900 East in Murray, Salt Lake County, Utah (Site or Basin Markets #105). Activities documented in this Work Plan were requested to be conducted by the Utah Division of Environmental Response and Remediation (DERR) in their July 3, 2024 letter. Additional Site investigation actions will include installation of four (4) groundwater monitoring wells (MW-23 through MW-26) and conduct of one groundwater monitoring and sampling event on Site associated wells. DERR requested the following scope of work in their July 3, 2024 letter, to be undertaken as part of this Work Plan: 1. Perform one (1) round of groundwater sampling for all the monitoring wells associated with this Site. 2. Install two (2) monitoring wells in the northern dispenser area to delineate the soil and groundwater contamination in this area. 3. Install one (1) monitoring well near the southwest corner of the Italian Village Restaurant, and another monitoring well (1) near the storm drain located on the east side of existing MW-11. 4. Collect representative soil samples from the four (4) borings during installation of the ne monitoring wells. 5. Analyze collected soil and groundwater samples for TPH-GRO, TPH-DRO, BTEXN, and MTBE by a Utah ELCP-certified laboratory. 6. Submit a monitoring well installation and groundwater sampling report that includes the following information: a. A comprehensive data table with all groundwater analytical results and depths to groundwater (historic and current). b. A comprehensive data table with all soil analytical results including sampling dates and sampling depths. c. Laboratory reports and chain-of-custody documentation including Utah DERR-certified sampler information. d. A groundwater gradient map depicting the groundwater flow direction. e. A figure depicting Site groundwater monitoring wells, and other relevant Site features. 1.1 Site Setting Basin Markets #105 is located in the northwest corner of the intersection of S 900 E and E 5400 S. The Site is bounded to the north by the Italian Village restaurant and a commercial parking lot, to the south by E 5400 S, to the east by S 900 E and to the west by the same commercial parking lot. The primary use of surrounding properties is commercial. It appears that the closest residential area is approximately 500 feet northeast of the Site. The Site is depicted in Figure 1. 1.2 Generalized Site Background The following is a generalized Site background summary. Semi-annual groundwater monitoring and sampling has been ongoing roughly at the Site since 2014. Twenty-two groundwater monitoring wells have been installed to monitor residual petroleum hydrocarbon impacts associated with the Site, including at Broadbent & Associates, Inc. Salt Lake City, Utah Amended Additional Investigation Work Plan Basin Markets #105/Murray October 17, 2024 Page 2 both on and off-Site locations. The locations of existing groundwater monitoring wells and other features associated with the Site are depicted on Figure 1. As of February 2021, the installed network of groundwater monitoring wells appears to have adequately delineated dissolved petroleum hydrocarbon perimeter impacts with the limited exclusion of specific areas that are the subject of this Work Plan. Residual impacts consist of dissolved-phase hydrocarbon mass of benzene and total petroleum hydrocarbons (TPH), both gasoline range organics (GRO) and diesel range organics (DRO), which are primarily observed in on-Site groundwater monitoring wells. Broadbent submitted the August 11, 2023 Corrective Action Plan (CAP) which proposed implementation of remedial action via ISCO methods utilizing DeepEarth Technologies, Inc., (DeepEarth) to design and implement their Cool-Ox® ISCO process at the Site. 2.0 ADDITIONAL SITE INVESTIGATION ACTIVITIES The purpose of this Work Plan is to address data gaps associated with residual subsurface impacts prior to consideration of remedial action. As previously stated, four (4) borings will be installed to facilitate collection of soil samples and will be completed as groundwater monitoring wells for future sampling. Review of Figure 1 indicates that new groundwater monitor wells will be installed in the following locations: one (1) monitoring well (MW-26) will be installed east of MW-11 and the adjacent storm water drain east of the Site’s c-store building; one (1) monitoring well (MW-23) near the southwest corner of the Italian Village Restaurant, and two (2) monitoring wells (MW-24 & MW-25) will be installed in the northern section of the fueling canopy east and west of previous boring B-2. Additional details regarding subsurface sampling and monitoring well installation are provided below. It is also recommended that three (3) existing monitoring well vaults be replaced due to damage. 2.1 Permitting and Utility Clearance Project setup activities will initially include contracting with a Utah licensed drilling contractor to perform well installation activities. Broadbent will coordinate the drilling schedule, and the driller will obtain appropriate permits as required. Broadbent will verify that an existing or a new access agreement will be completed prior to implementation of field work. Broadbent will mark the proposed drilling locations and ensure that the drilling contractor notifies Blue Stakes of the pending work a minimum of five (5) days prior to initiating field investigation. Broadbent will contract with a private utility locating company as additional measures for subsurface utility clearance. The borehole location will also be cleared to a depth of 6.5 feet bgs using a hand auger or air knife vacuum rig by the drilling contractor prior to borehole advancement. Finally, Broadbent will provide adequate notification of drilling activities to appropriate parties. 2.2 Health and Safety Plan Preparation Before work is initiated, an existing Health and Safety Plan (HASP) will be updated/modified for worker protection and hazard identification. The HASP will be Site specific and include the following at a minimum: • Site and task specific hazards; • Information on chemicals to be in use (SDS); • Directions and map to nearest hospital; and • Contact information for utilities, Pilot, and Broadbent. A copy of the HASP will be provided to the Utah State licensed drilling contractor to inform them of potential hazards associated with the Site and emergency procedures. Broadbent & Associates, Inc. Salt Lake City, Utah Amended Additional Investigation Work Plan Basin Markets #105/Murray October 17, 2024 Page 3 2.3 Soil Boring and Monitoring Well Installation Monitoring wells MW-23 and MW-26 will be installed off-Site while MW-24 and MW-25 will be installed on-Site. Broadbent has been contacted by other consulting companies regarding conduct of a Phase I environmental site assessment on the adjacent property and is assuming ownership has changed. With that stated, Broadbent will coordinate off-Site property access to verify approval for well installation. During drilling activities, soil cuttings will be described by the on-Site Broadbent representative using the Universal Soil Classification System (USCS) in accordance with ASTM D 2488-93 Standard Practice for Description and Identification of Soils (Visual-Manual Procedure). Soil cuttings will be examined using visual and manual methods for parameters including odor, staining, color, grain size, and moisture content. Drill cuttings will be monitored with a photo-ionization detector (PID), and placed in labeled 55- gallon drums, prior to temporary storage for ultimate disposal. Broadbent’s representative will also be a Utah-certified sampler and project oversight will be conducted under the direction of a Utah UST Consultant. The monitoring wells will be constructed of two (2)-inch diameter Schedule 40 PVC pipe. The total depth of each well is proposed to be approximately 13.5 feet bgs. The screen interval will consist of 10 feet of 0.010-inch factory slotted screen extending from approximately 3.5 feet bgs to approximately 13.5 feet bgs. Blank casing will extend from 3.5 feet bgs to just below land surface. Clean washed silica sand will extend from total depth drilled to approximately 3.0 feet bgs. Approximately 1.0 ft of bentonite clay will be placed above the sand interval and hydrated in place as part of the annular seal. Neat cement grout will be placed above the bentonite plug completing the surface seal. Wellheads will be secured with locking well caps and protected by traffic rated well vaults installed just below local land surface(s) to prevent damage. Three (3) existing monitor well vaults at MW-6, MW-8, and MW-12 will be replaced during this drilling event. 2.4 Soil Sampling The soil boring will be continuously cored, if possible, to assist with lithologic classification, PID monitoring, and soil sample collection. Soil samples will be collected for laboratory analysis at approximate intervals of five (5) feet. An additional sample will be collected at the groundwater interface based on depth to groundwater measured in adjacent existing wells. It is assumed that at least two (2) samples per boring will be collected for laboratory analysis. 2.5 Laboratory Analysis Collected samples will be submitted to Chemtech-Ford Laboratories in Sandy, Utah. Chemtech-Ford is a Utah State-certified environmental laboratory which will conduct the following analysis: GRO and DRO by EPA Method 8015B; Benzene, Toluene, Ethylbenzene, Total Xylenes, and Naphthalene (BTEXN) by EPA Method 8260B; and for Methyl Tert-Butyl Ether (MTBE) by EPA Method 8260B as well. 2.6 Well Development and Groundwater Sampling Broadbent will mobilize with DPS to initiate well development activities approximately 72 hours after well installation. Well development will consist of surging and pumping each well until relatively silt free water is removed. After development, the wells will be left to hydraulically equilibrate for approximately 48 hours prior to water level measurement and sampling. The new wells will be included in the subsequent Site-related groundwater monitoring event described below in Section. Broadbent & Associates, Inc. Salt Lake City, Utah Amended Additional Investigation Work Plan Basin Markets #105/Murray October 17, 2024 Page 4 2.7 Site Surveying New monitoring wells will be surveyed based on the existing Site datum by Broadbent personnel to support generation of a new Site figure. 2.8 Investigation-Derived Residuals Management Residual solids and liquids generated during the Site investigation activities will be stored temporarily on- Site in appropriately labelled Department of Transportation (DOT) approved 55-gallon drums pending analytical results and profiling. Drummed wastes will be scheduled for transport to an appropriate facility for treatment or disposal following the receipt of the certified laboratory analytical reports for analyses run on their contents. 3.0 GROUNDWATER MONITORING EVENT Broadbent will conduct a groundwater monitoring and sampling event utilizing 31 monitoring wells associated with the Site (MW-1 through MW-26, SV-1, EW-1, EW-2, EW-3, and A-1-1). Depth to groundwater and the presence of LNAPL will be measured in each well prior to sampling using an oil/water interface probe (OWIP). Monitoring wells containing a sufficient volume of groundwater and lacking measurable LNAPL will be purged and sampled. Monitoring wells suitable for sampling will be purged utilizing low-flow methods until general water quality parameters stabilize (temp., pH, conductivity, etc.). Purged groundwater from wells without measurable LNAPL will be poured over an impervious surface on Site and allowed to evaporate. Monitoring wells that do not already have dedicated tubing will have it installed from which groundwater samples will be collected in laboratory-provided volatile organic analysis (VOA) containers utilizing a submersible low-flow electric or peristaltic pump. The electric pump, if used, will be decontaminated prior to its use in each well. Samples will be submitted on ice, under chain-of-custody procedures, to Chemtech- Ford. Groundwater samples will be analyzed for the same constituents described above in Section 2.5. Site-associated monitoring wells with measurable LNAPL present will not be sampled but will be hand bailed utilizing a new poly bailer until either the well is bailed dry, measurable LNAPL is no longer detected, or until there is a consistent LNAPL thickness after three casing volumes of groundwater have been removed. Recovered LNAPL will be placed in 55-gallon drums and stored at the Site until enough is generated to warrant disposal. LNAPL disposal costs are not included in this Work Plan. 4.0 SITE INVESTIGATION AND GROUNDWATER MONITORING REPORT Broadbent will prepare a report summarizing Site investigation and groundwater monitoring activities as requested in DERR’s July 3, 2024, letter. The report will document fieldwork and laboratory analytical results and will include at a minimum the following information: • Description of field activities associated with Site investigation drilling, sampling, and groundwater monitoring • Monitoring well construction and lithologic logs • Comprehensive soil and groundwater analytical data tables • Site location and layout figures • Groundwater potentiometric surface and analytical results figures • Groundwater sampling data sheets • Project-related photographs (if required) • Laboratory analytical reports Broadbent & Associates, Inc. Salt Lake City, Utah Amended Additional Investigation Work Plan Basin Markets #105/Murray October 17, 2024 Page 5 • Discussion and recommendations 5.0 GENERAL PROJECT MANAGEMENT AND REIMBURSEMENT PACKET PREPARATION Broadbent will provide project management and oversight of field staff. A PST Fund Reimbursement Packet will be prepared after conduct of field work with a second submittal expected after submittal of the summary report. 6.0 SCHEDULE & COST Broadbent will make every attempt to carry out field work associated with this Work Plan; however, contractor availability may delay proposed activities. It is estimated that field work associated with well installation may require two (2-2.5) days based on Site-marking, utility location, monitoring well installation, and existing vault replacement. Well development and surveying are expected to require approximately one-half (0.5) day. Groundwater monitoring is expected to require up to three (3) days based on sampling of 31 monitoring wells. It is anticipated that a summary report will be submitted to DERR within 60-70 days after receipt of authorization to proceed in provided. Off- Site access for new monitoring well installation may require additional time. Anticipated costs associated with Work Plan activities are estimated at $32,521.45. These costs were developed based on the Utah PST Fund Guidelines. Preparations to conduct the proposed work will commence upon approval of this Work Plan. The total anticipated cost to complete project-related activities described herein is presented below. A detailed breakdown of costs is attached as Table 1. Broadbent will do its best to alert REH Marketing, Inc. of matters which, in the opinion of Broadbent require immediate attention to protect public health, safety, and environment. Broadbent will make every effort to advise REH Marketing, Inc. of matters which should be reported to government regulatory agencies. However, REH Marketing, Inc. is solely responsible for reporting such matters, and Broadbent shall not be held liable in the event that the proper agency is not notified. Our services will be performed in accordance with generally accepted practice at the time work commences. Results and recommendations will be based on review of available documentation and written or verbal correspondence with appropriate regulatory agencies, laboratory results, observations of field personnel, and the points investigated.
