Dear Kristi Bell:The Division of Drinking Water (the Division) received an exception request to R309-550-7(1) Minimum Separation Requirements for Valley Meadow Subdivision on September
3, 2024. The Division notified your consultant that the Valley Meadows Subdivision water line would require plan approval on September 6, 2024. The Division received the plans and specifications
for the proposed Valley Meadows Subdivision from your consultant, Matt Hartvigsen, P.E., with Jones & Associates on September 13, 2024. Written review comments were provided to your
consultant on October 7, 2024. The Division received plan review information including project specifications on October 8, 2924. This project has met the conditions for receiving a
plan approval.
Plan Approval
Our understanding of the project includes installing approximately 600 linear feet of 8-inch diameter C900 PVC pipe to serve nine new residential connections along 6750 South Street
in Uintah, UT. The project will also include installing two new fire hydrants.
We have completed our review of the plans and specifications, stamped, and signed byJames B. Flint, P.E., dated September 12, 2024, and find they basically comply with the applicable
portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans for the Valley Meadows Subdivision water line are hereby approved.
This approval pertains to construction only. An Operating Permit must be obtained from the Director before the Valley Meadows Subdivision water line may be put in service. A checklist
outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning
construction of this project. As the project proceeds, a notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water,
must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be
scheduled.
This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.
Exception to R309-550(7)(1) – Minimum Separation RequirementsAs part of the exception request included with this project, the Division received a Water/Sewer Separation - Exception Request
Form, a completed Water/Sewer Separation - Exception Request Spreadsheet detailing the five sewer/water crossing where minimum separation requirements cannot be met, and a drawing showing
the location of each crossing. The completed Exception Request Form is enclosed with this letter, constituting approval of the exception request.
Please maintain a copy of this letter with your permanent records for future reference.

Items 1 through 8 below must be submitted to the Division and found to be acceptable prior to operating permit issuance (unless a water line project meets the requirements of R309-500-7
and is not required to obtain an Operating Permit).
Distribution lines (not including in-line booster pump stations) requiring Plan Approval may be placed into service prior to submitting all of the following items and receipt of Operating
