Torrey, UT 84775 Subject: Notice of Violation – Capitol Reef Field Station Failure to Calibrate Turbidimeter at Field Station Well Membrane Filter (TP001), System #28042
Dear Harley Cinimin Kofford: Division records indicate Capitol Reef Field Station is in violation of the State of Utah Public Drinking Water Rule R309-215-9 as follows:
As a surface water treatment system, when operating and serving water to the public, Capitol Reef Field Station is required to report monthly system monitoring compliance, including turbidimeter calibration dates, for the Field Station Well Membrane Filter (TP001). Capitol Reef Field Station failed to calibrate turbidimeters for July 2024 (R309-215-8(1)(d)), despite operating in the month of July.
This violation results in the addition of 50 Improvement Priority Points to Capitol Reef Field Station’s Improvement Priority System Report (R309-400-5) as summarized below.
• Violation Type 10 (50 points) – Operations Report - System fails to calibrate turbidimeters
monthly [R309-215-9] (July 2024)
Return to compliance and removal of IPS points will be achieved by demonstrating two months of compliance through monthly Surface Water Treatment Reports for the Violation Type 10.
Because this is a Tier 3 violation, you must notify all consumers of the violation within one year of the violation according to R309-220-7. This public notice requirement cannot be waived once a violation has occurred. Public notification must be designed to reach all users. Water systems are to use, at a minimum, one or more of the following forms of delivery: broadcast media (such as radio and television), posting notice in a conspicuous location throughout the areas
served by water system, hand delivery, or another delivery method approved in writing by the

August 29, 2024 director. The public notice must be approved by the Division before it is distributed. Please refer to the enclosed example of a public notice for instructions and send a draft of the
public notice to Sarah Page at sepage@utah.gov for approval. Proof of public notification and
a copy of the notification must be provided to the Division at ddwreports@utah.gov and sepage@utah.gov within 10 days of notifying the water system customers. Please refer to the enclosed example of a public notice.
