Salt Lake City, UT 84150 Subject: NOTICE OF VIOLATION – Girls Home Failure to Maintain Microbial Treatment at Girls Home Filtration, TP001, System #02031
Dear Andrew Clark Beecher: Our records indicate that your system is in violation of the State of Utah Public Drinking Water Rules R309-200-7 and R309-215-10 as follows:
As a surface water treatment plant, the Girls Home water system is required to achieve giardia, virus, and cryptosporidium removal/inactivation of 3, 4, and 2-log, respectively. On July 15, 17, 19, 21, 23, 24, 25, 28, 29 and 31, 2024, the Girls Home Girls Home Filtration treatment plant failed to demonstrate the required treatment for virus inactivation.
Additionally, the Girls Home water system is required to notify the Division of Drinking Water (the Division) within 24 hours of a failure to demonstrate microbial treatment, and no such notification occurred.
Failure to demonstrate adequate treatment has resulted in the addition of 100 Improvement
Priority System (IPS) points to your IPS report (R309-400-5), as summarized in the list below.
• Violation type 41 (100 points) - Must be sufficient to ensure total treatment achieves
99.9% (3-log) inactivation and/or removal of Giardia and at least 99.99% (4-log)
inactivation and/or removal viruses as determined by the State. [R309-200-7, R309-215-10] (July 2024) Return to compliance and removal of IPS points will be achieved by demonstrating three months of compliance through monthly surface water treatment reports.

as soon as possible within 30 days. Upon receipt of the results, we will resolve the violation and
remove the points from your system’s IPS report. This Violation Type 41 is a Tier 2 violation. You must notify all consumers as soon as practical, but no later than 30 days after receiving notification of the violation according to
R309-220-6. This public notice requirement cannot be waived once a violation has occurred.
Public notification must be designed to reach all users, and the public notice must be approved by the Division before it is distributed. Please refer to the enclosed example of a public notice for instructions and send a draft of the public notice to Sarah Page at sepage@utah.gov for approval. Proof of public notification and a copy of the notification must be provided to the
Division at ddwreports@utah.gov and sepage@utah.gov within 10 days of notifying the water
