Dear Mr. Gilgen,
The New Source Performance Standards for Small Industrial-Commercial-institutional Steam
Generating Units (NSPS Boilers) 40 CFR 60 Subpart Dc reporting the quantity of fuel other than
natural gas combusted during the reporting period for Primary Children’s Medical Center Utah DAQ
Site ID 10461.
-Six-month reporting period covered under this is from January 1, through June 30, 2024.
-Boiler hours of operation on diesel fuel for the reporting period are Boiler #1 – 6 hours and
Boiler #2 – 6 hours for a total of 12 hours.
-The facility received two fuel deliveries for this period. A copy of the diesel certifications
from the fuel deliveries are included as reference.
In accordance with Utah Administrative Code R307-415-5d and based on information and belief
formed after reasonable inquiry, I certify that the statements and information in this document are
true, accurate, and complete.
