Dear Mr. Gilgen,
The New Source Performance Standards for Small Industrial-Commercial-institutional Steam
Generating Units (NSPS Boilers) 40 CFR 60 Subpart Dc reporting the quantity of fuel other than
natural gas combusted during the reporting period for Logan Hospital Utah DAQ Site ID 12377.
-Six-month reporting period covered under this is from January 1, through June 30, 2024.
-Boilers were not operated on diesel fuel during the reporting period.
-There were four fuel deliveries during the reporting period. ULSD certification is included as
an attachment.
In accordance with Utah Administrative Code R307-415-5d and based on information and belief
formed after reasonable inquiry, I certify that the statements and information in this document are
true, accurate, and complete.
