Dear Jamison Smith:The Division of Drinking Water (the Division) received your request for the Operating Permit for the Blackhawk Mountain Estates Well, Sundance Ridge Storage Tank and
Distribution System from your consultant, Steven M. Kamlowsky with Brown Consulting Engineers on September 23, 2024. The Division issued Plan Approval for drilling this well on August
23, 2017, and Plan Approval for equipping this well on October 17, 2022. The DEQ Central District Engineer conducted an onsite final inspection of this project on September 30, 2024.
Several items were noted in that inspection that needed to be addressed. Picture evidence of those items being corrected were submitted on October 3, 2024.
Our understanding of the project is that the Blackhawk Utilities, LLC water system is a new water system comprised of the following facilities:
Blackhawk Mountain Estates Well, identified as WS001 in the Division’s database:
8-inch diameter permanent well casing, drilled to a depth of 695 feet. Surface seal grout installed to a depth of 230 feet.
Equipped with a 30 HP submersible pump capable of producing 70 gallons per minute (gpm) at 1,000 feet of total dynamic head. The well was test pumped for 24 hours at a rate of 80 gpm
and had a dynamic drawdown to 345 feet.
A well house was constructed to house the wellhead and the well discharge piping, including a pump to waste line.
Sundance Ridge Storage Tank, identified as ST001 in the Division’s database:
A new 157,000 gallon steel storage tank was constructed to serve the current and future residential connections within the Blackhawk Utilities, LLC Water System.
Blackhawk Utilities, LLC Distribution System, identified as DS001 in the Division’s database:
The new distribution system is made up of approximately 18,000 feet of 8-inch diameter PVC piping and associated appurtenances; 2 pressure reducing valve stations (PRVs), isolation valves,
combination air-vacuum valves, fire hydrants and service laterals with individual meters.
We have received the following information for the Blackhawk Utilities, LLC Water System:
Design engineer’s statement of conformance with approval conditions.
Documentation of valid water rights.
As-built or record drawings.
Recorded land use agreements. Satisfactory bacteriological results as evidence of proper disinfection and flushing.
We have determined that all conditions of operating permit issuance have been met. On this basis, an Operating Permit for the Blackhawk Utilities, LLC Water System, including all facilities,is
hereby issued as constituted by this letter. You may now place the Blackhawk Mountain Estates Well (WS001), Sundance Ridge Storage Tank (ST001) and Distribution System (DS001)in service
in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
The equipped well pump capacity of the Blackhawk Mountain Estates Well is 70gpm. The safe yield of the Blackhawk Mountain Estates Well is rated at 53.33gpm, which is calculated based
on two-thirds of the constant-rate aquifer drawdown test results at 80gpm. The safe yield of 53.33 gpm is the basis for determining the maximum number of connections that the Blackhawk
Mountain Estates Well can serve.
Considering the safe yield of 53.33 gpm, themaximum number of residential connections (lots) that can currently be served by the Blackhawk Mountain Estates Well is 73 (lots). It is possible
to increase the number of lots to serve by performing another constant rate pump test at a higher rate or developing a second source of water. Division Rules do require a second source
of water once the number of connections served reaches 100 or more.
Updated Drinking Water Source Protection Requirement
The Division concurred with the final Drinking Water Source Protection (DWSP) plan in a letter dated April 14, 2022. The Division also received proof that land use agreement(s) had been
recorded in the County Recorder’s office on March 25, 2022. An updated DWSP plan is required on or before December 31, 2028. If you have questions about the source protection requirements,

Source Monitoring Requirements
Issuance of this Operating Permit changes your water system’s monitoring and reporting
requirements. The monitoring requirements for this well are given below. An updated monitoring schedule can be viewed any time at waterlink.utah.gov under the Water Monitoring section.
