Re:Lapsing of the Certificate of Compliance for Underground Storage Tanks 4, 5, & 6
Southway Mini Mart, located at 651 South Main Street, Blanding, Utah
Facility Identification No. 5000114
Dear Ms. Howe:
The Utah Underground Storage Tank Act (Act) requires that petroleum storage tank (PST)
owners and operators pay yearly PST registration fees, PST Fund fee, late penalties, and interest.
The Act states that if any of the assessments remain unpaid 60 days after the due date, the certificate
of compliance and coverage under the PST Fund, lapse.
Our records indicate that the assessments in the amount of $1,818.00 have not been paid and
are now more than 60 days past due.
Therefore, you are hereby notified that the PST Fund coverage and certificate of
compliance for Facility Identification Number 5000114, known as Southway Mini Mart, have
lapsed.
Without PST Fund coverage, expenses related to investigation or cleanup of any release of
petroleum from the tanks are ineligible for PST Fund reimbursement. Additionally, without a
certificate of compliance, the tanks cannot receive delivery of product.
To obtain a new certificate of compliance, you must do the following:
1. Document that you are in compliance with release detection requirements. Contact
our office for the specific leak detection records that are required for your facility;

2. Pay all past due registration fees;
3. Pay a $300.00 re-certification fee, and
4. Meet financial assurance requirements by either:
a. Reinstating PST Fund coverage, or
b. Documenting that you have an approved and current alternate financial
assurance mechanism.
Please be sure to monitor your own records to ensure compliance.
