HomeMy WebLinkAboutDDW-2024-011617 October 2, 2024 Bret Eldon Black Beaver City PO Box 271 Beaver City, Utah 84713 Subject:Operating Permit, Beaver City 2023 Culinary Water Improvements, 11,200 LF 8-inch water line and Tank Well House Modifications (WS011); Beaver City Water System, System #01001, File #13326, FSRF #3F1874 Dear Bret Black:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Beaver City 2023 Culinary Water Improvements from your consultant, Parker Vercimak, Jones & DeMille Engineering on September 27, 2024. Previously, the Division received the plans and specifications on March 30, 2023. Written review comments were provided to you and your consultant on April 20, 2023. The Division received the revised plans and specifications on April 25, 2023. The Division issued Plan Approval for this project on May 22, 2023. Our understanding of the project included the installation of approximately 11,300 linear feet of 8-inch diameter C900 PVC water pipe, fire hydrants, service connections and three river crossings along with all appurtenant valves and fittings. The project also included construction of a new well building, piping, electrical, and related work for the Tank Well (WS011). We have received the following information for the Beaver City 2023 Culinary Water Improvements: Certification of Rule Conformance with plan approval conditions by a professional engineer. As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Beaver City 2023 Culinary Water Improvementsis hereby issued as constituted by this letter. You may now place these facilities in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.
