Director March 18, 2024 Tracy Hobart Feltner Central Iron County WCD Bumblebee Water System 88 E Fiddlers Canyon Rd, Ste A Cedar City, UT 84721 Subject: Well Surface Seal Certification for the Vandenberghe Well (WS002), Bumblebee Water System, Water System #11089, File #12928 Dear Tracy Feltner On March 18, 2024, I witnessed the sealing procedure for the well referenced above as authorized by R309-515-6(5)(b). This letter is to certify that the well sealing procedure met the requirements of Utah Administrative Code (UAC), Rule R309-515-6(6)(i), Well Sealing Techniques and Requirements, and Rule R655-4-11.4, Surface Seals and Interval Seals. Well Background Well Name: Vandenberghe Well Water Right Number (based on Start Card issued by Division of Water Rights): 73-4481 Well Owner Name: Central Iron County Water Conservancy District (based on Driller StartCard) Well Type: Public Drinking Water Well PWS Name: Bumblebee Water System PWS Number (if applicable): 11089 Water Source ID (if applicable): WS002 DDW File Number (if applicable): 12928 Consultant Name and Company (if applicable): Ensign Engineering Well Location County: Iron County Nearest Town: Cedar City, UT Tracy Hobart Feltner March 18, 2024 Location listed on the Start Card: N 980’ W 2640’ from E4 Cor, S05 T37S R12W, SLB&M Local Health Department: Southwest Utah Public Health Department Street Address: Near 3400 S 8200 W, Cedar City, UT Latitude/Longitude: N 37.612398 deg W 113.22552 deg Method Used to Determine Latitude & Longitude: Handheld GPS (OnX GPS App) Datum: NAD83 Well Head Elevation: 5,545’ Method Used to Determine Elevation: Google Earth Well Driller Name of Licensed Well Driller: Melvin Dale Gardner License Number: 492 Company Representative’s Name: Trevor Gardner Company Name: Gardner Bros Drilling Mailing Address: PO Box 965, Enterprise, UT 84725 Phone Number: 435-878-5602 Responsible driller/operator onsite during well sealing: Trevor Gardner Well Construction Well Drilling Method: Well was previously drilled but only had a 20-foot surface seal. Driller used a 6-inch bit around the existing 16-inch permanent casing to create an annulus between the casing and borehole to a depth of about 106 feet so a grout seal could be installed Casing Types Installed: Permanent casing Casing Diameter: 16” Casing Length: 458’ Casing Material: Steel Annulus Size (between borehole and casing): Approximate 6 inches (28-inch borehole) Pitless Adapter: No . Well Sealing Description Description of the Well Sealing Process: The well was grouted using neat cement grout and pumped through a 2-inch tremie to the bottom of the borehole at a depth of 106 feet. Grout Material: Neat Cement Grout Volume (in cu. yds.): approx. 20+ Cu Yards. The calculated required amount was about 11.3 CY. Grout Density (in lbs/gal): 16, 15.5, and 15.5 lbs/gal. (three trucks with about 10 yards each) Method of Placement: Positive displacement grout pump Depth from Surface (in feet): 106’ Did Grout Come to the Surface? Yes, it came to the surface, Length of tremie pipe: 100’ Tracy Hobart Feltner March 18, 2024 Names & Affiliations of Other Observers Brett Gardner – Gardner Bros Drilling Trevor Gardner – Gardner Bros Drilling Steve Adams – Pump contractor Issues Observed during Well Sealing No issues were observed. However, the first cement truck came with a batch mix of about 20,000 lbs of cement, but only about 680 gallons.of water. While it weighed over 15 lbs per gallon, it was difficult to pump, even after adding more water and it was starting to set up. After using about half of the truck, it had to be sent back. The next two trucks both came with 20,000 lbs of cement and about 1,000 gallons of water. The full second truck (10 CY) was used and over half of the third truck. This well seal certification is not a Plan Approval to construct a public drinking water well or an Operating Permit to use the well as a public drinking water source. Such approval may only be granted by the Director of the Division of Drinking Water. A copy of this letter has been sent to the Director of the Division of Drinking Water.
