Dear Todd Poulsen:
Skyline Mine is required to conduct increased sampling at Well #1 (WS001) to investigate the quality of that
source.
1. With system name here request to uninstall treatment, this source water quality evaluation has added the
following monitoring requirement for your system:

2. Treatment cannot be uninstalled until this sampling has been completed. The sampling requirements
presented above will remain in place for a minimum of four quarters.
3. All bacteriological source samples shall be collected prior to any treatment and/or at a location that
represents the source water quality.
4. The intent of the iron, manganese, turbidity and TDS samples is to determine how filtration may impact
the concentration of these parameters. Please collect the samples listed above.
a. Well #1 (WS001) samples shall be collected directly from the well head, upstream of any
treatment.
b. WQTP & CL2 (TP001) samples shall be collected immediately downstream of the filter at a
location that represented the water after it has undergone treatment, and before it has entered the
distribution system,
5. Label all source samples with the facility name and code listed above as the sampling location and
facility code.
Please note that this bacteriological source sampling requirement is in addition to the distribution system
bacteriological sampling requirement already in place. All distribution samples must be labeled with sample
location/sample address and DS001 for the facility code.

and then selecting Water Monitoring under Water System Reports.
