INTRODUCTION
Wasatch Environmental, Inc., (Wasatch) is pleased to submit this monitoring well repair report detailing
the monitoring well MW-9 surface completion repair activities conducted for the former American Food
Mart property (Site) release located at 1605 South Foothill Drive in Salt Lake City, Utah. See Figure 1 for
a monitoring well location map.
MONITORING WELL REPAIR ACTIVITIES
On July 10, 2024, Wasatch personnel removed the damaged MW-9 surface completion that is located
within the 1700 South roadway (see Appendix A for a photolog of activities). Prior to removal of the well
surface completion, Wasatch obtained a Salt Lake City (SLC) Traffic Control Permit and Work in the
Public Way Permit (see Appendix B for a copy of the SLC permits obtained). The concrete apron and
broken well vault were removed from the ground surface and disposed at the SLC landfill. Wasatch
installed a new 12-inch flush-mounted, traffic-rated, steel vault set in concrete.
Once the MW-9 repairs were completed, traffic cones were placed on each side of the well to prevent
damage to the curing concrete. However, an unknown person removed the cones and drove on the
newly repaired surface. This resulted in the well vault being depressed forming a pothole hazard and
ruining the concrete finish. An SLC inspector informed Wasatch of the damage during his final inspection
of the work. Wasatch notified the Division of Environmental Response and Remediation (DERR)
Petroleum Storage Tank (PST) Fund that the repair work had been damaged, and SLC was requiring
Wasatch to re-complete the repairs. The DERR approved a separate work plan to complete the second
round of well surface repairs.
Wasatch was required to obtain a second traffic control plan to complete the second round of repairs.
The second round of repairs was completed on August 23, 2024, as described above. The SLC inspector
has inspected and approved of the second round of repairs. Therefore, the well repair activities are
complete.
Traffic controls were provided by Utah Barricade as specified on the SLC permits for each round of
repairs.

warranties either expressed or implied.
