HomeMy WebLinkAboutDDW-2024-010757 September 9, 2024 Geno Defa Defa’s Dude Ranch PO Box 12 Hanna, Utah 84031 Subject:(Conditional) Plan Approval, Rehabilitate Existing Well (WS001), System #07029, File #16343 Dear Geno Defa:The Division of Drinking Water (the Division) received the plans and specifications for the subject project from your consultant, Jones and Demille Engineering, on August 30, 2024. The Division issued review comments on September 4, 2024, and received updated plans and specifications on September 5, 2024. The project consists of cleaning and rehabilitating the well casing and screens and pump, installing a pitless adapter to raise the wellhead above the finish grade and replacing the connecting piping and appurtenances. The Division has completed its review of the plans and specifications, stamped, and signed by Eric L. Major, P.E., and dated September 5, 2024, and find that with the conditions outlined below they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, we hereby approve the proposed plans to rehabilitate the well, subject to the following conditions: Modify the plans to show the end of the downturned air/vac valve discharge piping a minimum of 1 foot above finished grade Clarify that both the fence and upgrading the vault to have a locking door are not required, either one meets the secure wellhouse requirements. Conditions of approval must be addressed in writing to the satisfaction of the Director with the request for an Operating Permit. This conditional approval pertains to construction only. An Operating Permit must be obtained from the Director before the rehabilitated well may be put back in service. A checklist outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.
