Box 148250 Salt Lake City, Utah 84114 Subject:Permanent Operating Permit, Oak Spring Redevelopment (WS001); Oak Springs Highway Rest Stop, System #21019, File #12213 Dear Ryan Ellsworth:The Division of Drinking Water (the Division) received your request for a Permanent Operating Permit for the Oak Spring Redevelopment Project from you on September 4, 2024. The Division issued a Temporary Operating Permit for this project on August 30, 2021. The Department of Environmental Quality Central District Engineer conducted an onsite final inspection of this project on June 29, 2021.Our understanding of the project is that the Oak Spring collection system and spring box has been replaced. Redeveloping the spring also included removing all of the deep-rooted vegetation in the spring collection area, removing and replacing the fence and installing a 40-mil impermeable liner over the collection system. The project also replaced the existing Oak Springs Tank (ST001) that originally served as a storage tank and spring collection box. The Temporary Operating Permit issued in 2021 was conditional upon you collecting 3 years of monthly spring flow data and submitting that data before November 1, 2024. That spring flow data has been received and reviewed to establish the safe yield of Oak Spring (WS001). We previously received the following information for the Oak Spring Redevelopment:Design engineer’s statement of conformance with approval conditions. As-built or record drawingsSatisfactory bacteriological results as evidence of proper disinfection and flushing Operating Permit for Oak Spring (WS001) We have determined that all conditions of issuing a Permanent Operating Permit have been accomplished. On this basis, a Permanent Operating Permit for Oak Spring is hereby issued as constituted by this letter. Please maintain a copy of this letter with your permanent records for future reference.The yield of Oak Spring (WS001) is determined to be 24.3 gallons per minute (gpm) based on the spring flow information we received. This yield is the basis for determining the maximum number of connections that the Oak Spring can serve.The issuance of this Operating Permit resolves your water system’s deficiency related to submitting 3 years of flow data from Oak Spring (WS001).
