To property owner:
The Utah Department of Environmental Quality (UDEQ), Division of Environmental
Response and Remediation (DERR) is planning an effort to evaluate potential groundwater and soil
gas impacts related to historic dry-cleaners near 900 South 200 West in Salt Lake City. The UDEQ
has identified your property as a location that can provide useful information for determining
potential impacts from these historic dry-cleaning operations. The UDEQ, in conjunction with the
U.S. Environmental Protection Agency (EPA), plans to conduct groundwater and soil gas sampling
at your property and other properties in the vicinity during Summer 2023. This sampling will be
performed at no cost to the property owner and UDEQ will notify property owners when the
laboratory results are available.
A standard consent for access form documenting your consent for sampling your property
is enclosed with this letter. Please sign and return the enclosed form in the enclosed stamped
envelope by July 13, 2023. In addition, please be sure to provide a daytime contact phone
number and an email address so field crews can reach you in a timely fashion once the
sampling schedule is confirmed. The grant of access can also be scanned and emailed to
etagliaferri@utah.gov and/or tdaniels@utah.gov.

A. I, the authorized representative of the owner of the property described above
(“Owner”), consent to officers, employees, contractors, subcontractors, and other authorized
representatives of the United States Environmental Protection Agency (“EPA”) and the Utah
Department of Environmental Quality (“UDEQ”) entering and having continued access to the
above referenced (“Property”) for the following purposes:
1. Conducting field inspections and investigations;
2. Taking samples of surface soil, subsurface soil, air, or water on the Property;
3. Drilling, boring or punching of holes for surface and subsurface investigations; and
4. Other such actions as may be necessary to protect human health and the environment.
B. Section 19-6-304 of the Utah Code provides that, upon request, the Owner may
have a split sample if possible and may obtain an analysis of the sample results. It is the Owner’s
responsibility to provide the necessary sample containers to obtain splits and arrange for the
analysis and cost of the analysis of the split samples
C. When available, UDEQ will provide the Owner with a copy of the analytical
results and laboratory data for samples taken from the property. Please indicate where the results
