Subject:Operating Permit, 5600 South Culinary Pipeline Relocation (Phase 1);

Dear Darren Hess:The Division of Drinking Water (the Division) received a request for an Operating Permit for 5600 South Culinary Pipeline Relocation (Phase 1) from your consultant,
Tanner Sweat, P.E. with Bowen Collins & Associates on September 3, 2024. The Division issued Plan Approval for this project on November 14, 2023.
Our understanding of the project was the relocation of two segments of existing culinary water
pipeline infrastructure due to impacts of the UDOT 5600 South Interchange project. Approximately 2,300 feet of 24-inch diameter ductile iron pipe (DIP) and 2,600 feet of 30-inch
diameter Prestressed Concrete Cylinder Pipe (PCCP) was replaced by 4,900 feet of 30-inch
diameter DIP. Another 500 feet of 16-inch diameter DIP was replaced with 30-inch diameter
HDPE. Two air valves were replaced and another five were added along with the replacement
of four blowoff valves and three isolation valves.
We have received the following information for 5600 South Culinary Pipeline Relocation (Phase 1):
Certification of Rule Conformance with plan approval conditions by a professional engineer.
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Evidence of O&M manual delivery.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for 5600 South Culinary Pipeline Relocation (Phase 1)is hereby
issued as constituted by this letter. You may now place 5600 South Culinary Pipeline Relocation (Phase 1) in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
