This Work Plan was requested by the Utah Department of Environmental Quality (UDEQ), Division of Environmental Response and Remediation (DERR), under Contract #216383, Work Assignment JDI-12-TER. The objective of this Work Plan is to secure permission to access the site wells, conduct one round of monitoring and sampling of all available monitoring wells, and submit a report documenting the monitoring event. It is anticipated site access can be acquired by calling or emailing the property owners and that they will provide access and sign a DERR Grant of Access form without issue. If the site owner, or occupant if different than the owner, resists site access, we will contact you to discuss alternative actions that are beyond the scope of this Work Plan. Subtotal for Task 1: .................................................................. $395.00 Task 2: Groundwater Monitoring and Sampling Terracon utilized the Utah DEQ’s Interactive Map to confirm the most recent documents associated with the former Beehive Credit Union property. According to the available records, the most recent document is the Groundwater Monitoring Report dated December 17, 2019, completed by Terracon Consultants, Inc. The findings of the 2019 report identified that four on-site and one off-site monitoring wells have historically been sampled for analysis of methyl tert-butyl ether, benzene, toluene, ethylbenzene, total xylenes, and naphthalene (MBTEXN); total petroleum hydrocarbons in the gasoline range (TPH-GRO); and total petroleum hydrocarbons in the diesel range utilizing silica gel treatment (TPH-DRO with SGT). Additional monitoring wells were historically installed at the site; however, they are assumed to no longer exist. As far as Terracon is aware, these five monitoring wells remail viable for sampling (MW-1, MW-2, MW-3, MW-B, and MW-D). The attached Exhibit 1, extracted from the 2019 report, shows the locations of the monitoring wells. Terracon will use historical site maps to confirm if other monitoring wells viable for sampling exist. If Terracon identifies such monitoring wells, the DERR project manager will be contacted prior to sampling to get approval for the additional scope of work and fee. The groundwater monitoring event will include documenting depth to water, thickness of any free product, if encountered, other observations, and collecting groundwater samples for laboratory analyses from five groundwater monitoring wells. The samples will be collected by a Utah-certified Soil & Groundwater Sampler following applicable DERR regulations and guidance. The groundwater samples will be submitted under chain-of-custody protocols to Chemtech Ford Analytical Laboratory for analysis of: All five monitoring wells: MBTEXN, TPH-GRO, and TPH-DRO by EPA Method 8260/8015M; Monitoring wells MW-B and MW-D:TPH fractionation by EPA Method 8270. Subtotal for Task 2: ............................................................... $2,578.95 Work Plan JDI-12-TER JDI-12-TER (Facility ID 4002050 / Release ID JDI) | Salt Lake City, Utah June 25, 2024 | Terracon Project No. AL137352 2 Task 3: Project Documentation, Report, and PST Claims Management Upon completion of site activities and receipt of the laboratory analytical results, a draft Groundwater Monitoring Report will be prepared and submitted to DERR that will include the following. Documentation of field activities Site plan showing pertinent site features Current and historical laboratory analytical laboratory results Current and historical groundwater elevation data A groundwater gradient map Iso-concentration contour maps for TPH-DRO and TPH-GRO, the two constituents where iso-concentration maps were prepared in the last sampling event conducted in 2019. Data evaluation and presentation of findings A final Groundwater Monitoring Report will be generated and submitted following receipt of comments or revisions requested by DERR on the draft report. For the purposes of this budget, we have assumed that no significant changes will be requested. Task 3 also includes time to generate the PST Fund invoices and coordination to complete the invoice with the client and DERR. Terracon makes no warranties, expressed or implied, regarding its services, findings, conclusions or recommendations. Please note that Terracon does not warrant the work of laboratories, regulatory agencies or other third parties supplying information used in the preparation of the report. If PST Fund Labor rates are changed prior to work being completed, the Work Plan will be updated to reflect the new rates. Terracon will not exceed the estimated costs without prior approval from the DERR Project Manager. ASSUMPTIONS The following assumptions were made during preparation of this Work Plan and Price: Terracon assumes that no on- or off-site access issues will be encountered. In the event Terracon encounters difficulty with access to the site or adjoining property, DERR will be contacted immediately to determine appropriate courses of action. Terracon assumes that a total of five monitoring well locations will be gauged and sampled. If more or fewer well locations are accessible for sampling, DERR will be contacted immediately to determine appropriate courses of action. Terracon assumes that the monitoring wells proposed for sampling are in adequate working condition with sufficient water column to allow sampling and are not covered by asphalt or otherwise inaccessible. For cost estimating purposes, Terracon assumes five groundwater samples will be submitted to the analytical laboratory, one from each of the 5 wells.
