COX Governor DEIDRE HENDERSON Lieutenant Governor August 6, 2024 Andrew Clark Beecher Camp Zarahemla 50 E North Temple St, Rm 1205 Natural Resources Services Salt Lake City, UT 84150 Subject: NOTICE OF VIOLATION – Camp Zarahemla (#15023) Failure to Report Turbidity Entering the Distribution System at Camp Zarahemla Treatment Plant (TP001) Dear Andrew Clark Beecher: The Division of Drinking Water records indicate that Camp Zarahemla (#15023) is in violation of the State of Utah Public Drinking Water Rule 309-215-9(1) as follows: Camp Zarahemla (#15023) is required to continuously monitor for turbidity at the treatment plant’s combined filter effluent (CFE) or clearwell outlet. Camp Zarahemla (#15023) must report the maximum turbidity value for each 4-hour operational period in the monthly report, submitted by the 10th of the following month. Camp Zarahemla Camp Zarahemla Treatment Plant June 2024 Water Treatment Operation Reports was missing records of CFE turbidity on June 4th and 6th of 2024. Failure to provide adequate records of CFE turbidity has resulted in the addition of has resulted in the addition of 50 Improvement Priority System (IPS) points to your IPS report (R309-400-5) as summarized below. • Vio 10 (50 points) – System fails to meet reporting and recordkeeping requirements [309-215-9] (June 2024) Return to compliance and removal of IPS points will be achieved by demonstrating two months of compliance through monthly Surface Water Treatment Reports. Because this is a Tier 3 violation, you must notify all consumers of the violation within one year of the violation according to R309-220-7. This public notice requirement cannot be waived once a violation has occurred. Public notification must be designed to reach all users. Water systems are to use, at a minimum, one or more of the following forms of delivery: broadcast Andrew Clark Beecher Page 2 of 2 August 6, 2024 media (such as radio and television), posting notice in a conspicuous location throughout the areas served by water system, hand delivery, or another delivery method approved in writing by the director. The public notice must be approved by the Division before it is distributed. Please refer to the enclosed example of a public notice for instructions and send a draft of the public notice to Sarah Page at sepage@utah.gov for approval. Proof of public notification and a copy of the notification must be provided to the Division at ddwreports@utah.gov and sepage@utah.gov within 10 days of notifying the water system customers.
