SUBJECT: No Further Action Leaking Petroleum Storage Tank - Closure Review Memo
Maverik #314 located at 795 South State Street, Orem, Utah
Facility Identification No. 1000371, Release Site OIS
A petroleum release from an underground pipe was reported to the Division of Environmental Response
and Remediation (DERR) on September 6, 2024. The release was reported to the DERR based on a
failed line tightness test. The extent and degree of contamination has been fully defined and a No Further
Action (NFA) review is hereby requested.
Due to residual contamination that is to remain in place, this release was discussed at Closure
Committee on August 21, 2024. In this meeting it was decided that the DERR will utilize Blue Stakes
of Utah and create a contamination map to supplement the NFA letter to alert excavators who may
work in the area.
